摘 要:随着肥胖和2型糖尿病发病率的迅速上升,非酒精性脂肪性肝病(nonalcoholic fatty liver disease,NAFLD)现已成为我国最常见的慢性肝病。NAFLD是一种以肝细胞脂肪变性为主要特征的临床病理综合征,其主要病理过程包括单纯性脂肪肝、非酒精性脂肪性肝炎(nonalcoholic steatohepatitis,NASH)和肝纤维化,进而发展成肝硬化及肝细胞癌。然而,NAFLD的发病机制尚未完全明确,也无相应的有效治疗措施。Hedgehog(Hh)信号通路是伤口愈合反应的主要调控者,在健康肝脏中处于静止状态,一旦肝脏受损该信号通路即被激活。适当激活Hh信号通路可抑制NAFLD初期阶段的肝脂肪变性;过度活化的Hh信号通路则促进NASH、肝硬化和原发性肝癌的发生发展;抑制过度的Hh信号通路活性是防止NAFLD逐渐恶化的一种潜在方法。本文对Hh信号通路在NAFLD发生与发展中的作用进行综述并讨论Hh信号通路靶向干预在预防NASH、肝硬化和肝癌中的潜在作用。With the rapid increase in the incidence of obesity and type 2 diabetes,nonalcoholic fatty liver disease(NAFLD)has become the most common chronic liver disease in China.NAFLD is a clinicopathological syndrome characterized by hepatocellular steatosis,and its main pathological processes include simple fatty liver disease,nonalcoholic steatohepatitis(NASH)and liver fibrosis,which in turn develop into cirrhosis and hepatocellular carcinoma.However,the pathogenesis of NAFLD has not been fully elucidated,and there is no corresponding effective treatment.The Hedgehog(Hh)signaling pathway,which is the major regulator of wound healing response,is quiescent in healthy liver and is activated once the liver is damaged.Appropriate activation of Hh signaling pathway can inhibit hepatic steatosis in the early stage of NAFLD,and over-activation of Hh signaling pathway can promote the occurrence and development of NASH,liver cirrhosis and primary hepatic cancer.This article reviewed the role of Hh signaling pathway in the occurrence and development of NAFLD and discusses the potential role of Hh signaling pathway-targeted intervention in the prevention of NASH,liver cirrhosis and liver cancer.
