摘　　要：目的探讨低分割调强放疗在乳腺癌保乳术后患者中的应用效果。方法选取2021年4月至2023年6月于烟台市烟台山医院行保乳术治疗的103例乳腺癌患者的临床资料进行回顾性分析,根据治疗方式分为低分割组(n=64,采取低分割调强放疗)与常规组(n=39,采取常规放疗治疗),比较两组应用效果。结果治疗后,两组T淋巴细胞亚群水平均下降,低分割组CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)高于常规组,CD8^(+)低于常规组(P<0.05)。治疗后,两组肿瘤标志物水平均下降,且低分割组糖类抗原15-3、糖类抗原125、甲胎蛋白、癌胚抗原水平均低于常规组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论低分割调强放疗在乳腺癌保乳术后患者中应用,有助于改善T淋巴细胞亚群及肿瘤标志物水平。Objective To explore the application effect of low-segmentation intensity modulated radiotherapy in patients with breast cancer after breast conserving surgery.Methods The clinical data of 103 patients with breast cancer who underwent breast conserving surgery in Yantai Yantaishan Hospital from April 2021 to June 2023 were retrospectively analyzed.According to the treatment methods,the patients were divided into the low-segmentation group(n=64,treated with low-segmentation intensity modulated radiotherapy)and the conventional group(n=39,treated with conventional radiotherapy).The application effects of the two groups were compared.Results After treatment,the levels of T lymphocyte subsets in both groups decreased,and the CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the low-segmentation group were higher than those in the conventional group,while the CD8^(+)was lower than that in the conventional group(P<0.05).After treatment,the levels of tumor markers in both groups decreased,and the levels of carbohydrate antigen 15-3,carbohydrate antigen 125,alpha fetoprotein,and carcinoembryonic antigen in the low-segmentation group were lower than those in the conventional group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The application of low-segmentation intensity modulated radiotherapy in patients with breast cancer after breast conserving surgery is helpful t o improve the levels of T lymphocyte subsets and tumor markers.
