摘  要:目的探讨高分辨率CT(HRCT)及MRI对感音神经性耳聋(SNHL)患者内耳畸形的临床诊断价值。方法回顾性收集2016年12月至2021年12月因SNHL就诊并于西安交通大学附属西安市中心医院行HRCT检查的82例SNHL患者的影像资料,其中37例行MRI检查。评估患者的内耳结构异常、蜗神经发育及面神经管异常情况。结果82例患者共142耳表现为内耳畸形,其中2耳Michel畸形,3耳耳蜗未发育,4耳共同腔畸形,5耳耳蜗发育不良,4耳不完全分隔Ⅰ型(IP-Ⅰ),61耳不完全分隔Ⅱ型(IP-Ⅱ),6耳不完全分隔Ⅲ型(IP-Ⅲ),28耳前庭、半规管畸形,26耳前庭导水管扩大,3耳内听道畸形。67耳行MRI检查,26耳(38.81%)伴蜗神经发育不良(CND)。142耳中有28耳(19.72%)伴面神经管异常。结论SNHL患者内耳畸形种类多样,CND及面神经管异常的发生率均较高。术前影像学检查可以为进一步人工耳蜗植入术提供有价值的信息。Objective To explore the clinical value of high-resolution CT(HRCT)and MRI in the diagnosis of inner ear malformation in patients with sensorineural hearing loss(SNHL).Methods The imaging data of 82 patients with SNHL who underwent HRCT examination in Xi'an Central Hospital Affiliated to Xi'an Jiaotong University from December 2016 to December 2021 were retrospectively collected,and 37 of them underwent MRI examination.The patients'inner ear structural abnormalities,cochlear nerve development and facial nerve canal abnormalities were evaluated.Results A total of 142 ears of 82 patients showed inner ear malformation,including 2 ears of Michel deformity,3 ears of underdeveloped cochlea,4 ears of common cavity malformation,5 ears of cochlear dysplasia,4 ears of incomplete partition typeⅠ(IP-Ⅰ),61 ears of incomplete partition typeⅡ(IP-Ⅱ),6 ears of incomplete partition typeⅢ(IP-Ⅲ),28 ears of vestibuleand semicircular canal malformation,26 ears of enlarged vestibular aqueduct,3 ears of internal auditory canal malformation.MRI examination was performed in 67 ears,and 26 ears(38.81%)had cochlear nerve deficiency(CND).Among 142 ears,28 ears(19.72%)had facial nerve canal abnormalities.Conclusion There are various types of inner ear malformations in SNHL patients,and the incidences of CND and facial nerve canal abnormalities are relatively high.Preoperative imaging examination can provide valuable information for further artifical cochlear implantation.
