IDH1基因表达与脑胶质瘤的MRI表现及ASL的关系

摘　　要：目的研究异柠檬酸脱氢酶1(IDH1)基因表达与脑胶质瘤的MRI表现及动脉自旋标记(ASL)的关系。方法回顾性分析48例病理证实的脑胶质瘤患者[其中IDH1(+)20例,IDH1(-)28例]的MRI及ASL。分析IDH1(+)与IDH1(-)脑胶质瘤的MRI;分析病灶水肿、出血、囊变、钙化、强化情况及ASL。结果IDH1(+)非胶质母细胞瘤中,水肿、出血、囊变及强化程度、ASL高灌注占比明显高于IDH1(-)非胶质母细胞瘤(P<0.05)。IDH1(+)胶质母细胞瘤中,囊变占比明显低于IDH1(-)胶质母细胞瘤(P<0.05)。结论不同IDH1基因表达脑胶质瘤的MRI及ASL表现不同,从胶质瘤的MRI表现可以预测IDH1基因突变情况。Objective To study the relationship between isocitrate dehydrogenase 1(IDH1)gene expression and MRI findings and arterial spin labeling(ASL)in brain glioma.Methods The MRI and ASL of 48 patients with pathologically confirmed brain glioma[20 cases of IDH1(+)and 28 cases of IDH1(-)]were retrospectively analyzed.The MRI of IDH1(+)and IDH1(-)brain glioma was analyzed;the edema,hemorrhage,cystic degeneration,calcification,enhancement and ASL of the lesions were analyzed.Results In IDH1(+)non-glioblastoma,the proportion of edema,hemorrhage,cystic degeneration,enhancement and ASL high perfusion were significantly higher than those of IDH1(-)non-glioblastoma(P<0.05).In IDH1(+)glioblastoma,the proportion of cystic degeneration was significantly lower than that in IDH1(-)glioblastoma(P<0.05).Conclusion The MRI and ASL manifestations of brain glioma with different IDH1 gene expression are different.The mutation of IDH1 gene can be predicted from the MRI manifestations of brain glioma.
