摘　　要：目的 探讨结肠减压后择期手术治疗结肠癌合并急性肠梗阻患者的效果。方法 将2022年1月—2023年12月在本院治疗的74例结肠癌合并急性肠梗阻患者随机分为两组,每组37例。对照组采用常规清肠+置入结肠支架治疗,观察组实施结肠减压后择期手术,对比两组的各项手术指标、并发症、炎症因子。结果 两组手术时间相当(P>0.05),但观察组淋巴结清扫数目多于对照组,术中出血量少于对照组,术后肛门排气时间、开始流质饮食时间、术后住院时间均短于对照组(P<0.05);观察组炎性肠梗阻、切口感染、肺部感染、吻合口漏等并发症发生率低于对照组(P<0.05);观察组术后3 d的IL-6、IL-8、TNF-α水平均低于对照组(P<0.05)。结论 结肠减压后择期手术治疗结肠癌合并急性肠梗阻患者的效果显著,能提高淋巴结清扫数目,减少术中出血量,降低并发症发生率和术后炎症水平,加快术后恢复。Objective To explore the effect of selective operation after colon decompression on patients with colon cancer complicated with acute intestinal obstruction.Methods Seventy-four patients with colon cancer complicated with acute intestinal obstruction who were treated in our hospital from January 2022 to December 2023 were randomly divided into two groups.The control group was treated with routine bowel cleaning and colon stent placement,and the observation group was treated with elective surgery after colon decompression.The surgical indexes,complications and inflammatory factors of the two groups were compared.Results The operation time of the two groups was similar(P>0.05),but the number of lymph node dissection in the observation group was more than that in the control group,and the amount of blood loss during operation,the time of anal exhaust after operation,the time of starting liquid diet and the postoperative hospitalization time were less than those in the control group(P<0.05).The incidence of complications such as inflammatory intestinal obstruction,incision infection,lung infection and anastomotic leakage in the observation group was lower(P<0.05).The levels of IL-6,IL-8 and TNF-αin the observation group were lower on the 3rd postoperative day(P<0.05).Conclusion Selective operation after colon decompression is effective in the treatment of patients with colon cancer complicated with acute intestinal obstruction,which can increase the number of lymph node dissection,reduce the amount of intraoperative bleeding,reduce the incidence of complications,reduce the level of postoperative inflammation and speed up postoperative recovery.
