鼻胆管短单反“α”固定方法的循证护理实践

摘　　要：目的 通过循证护理实践寻找鼻胆管的最佳固定方法。方法 选择2021年5月—2023年5月本院收治的行经内镜鼻胆管引流术(Endoscopic Nasobiliary Drainage,ENBD)的患者42例作为研究对象,比较对照组(采用传统蝶形平行固定法)和观察组(采用基于循证的鼻胆管短单反“α”固定法)的临床效果。结果 两组患者鼻胆管固定后均未出现中途拔管、脱管和黏膜出血。两组患者性别、年龄、文化程度、胆总管直径、鼻胆管固定时间、住院时间、C反应蛋白水平对比,差异无统计学意义(P>0.05)。与对照组相比,观察组患者48 h胆汁引流量显著增加(P<0.05);观察组术后胆红素、体温比术前显著下降(P<0.05);对照组和观察组术后疼痛等级均显著下降(P<0.05)。结论 基于循证的鼻胆管短单反“α”固定法有助于加强患者胆汁引流效果,降低胆红素水平,缓解疼痛,值得临床护理推广。Objective To find the optimal fixation method for nasobiliary tract through evidence-based nursing practice.Methods 42 patients who underwent endoscopic nasobiliary drainage in our hospital from May 2021 to May 2023 were selected as the study subjects.The clinical effects of the control group(using traditional butterfly parallel fixation method)and the observation group(using evidence-based nasobiliary short reflex"alpha"fixation method)were compared.Results Both groups of patients did not experience mid course extubation,dislodgement,or mucosal bleeding after nasobiliary fixation.There was no statistically significant difference in gender,age,education level,common bile duct diameter,nasobiliary fixation time,length of hospital stay,and C-reactive protein levels between the two groups of patients.Compared with the control group,the observation group showed a significant increase in bile drainage volume within 48 hours(P<0.05);The observation group showed a significant decrease in postoperative bilirubin and body temperature compared to preoperative levels(P<0.05);Both the control group and the observation group showed a significant decrease in postoperative pain levels(P<0.05).Conclusion The evidence-based“α”fixation method for short nasal bile ducts is beneficial for improving bile drainage efficiency,reducing bilirubin levels,and relieving pain in patients,and is worthy of clinical nursing promotion.
