摘　　要：目的 探讨注射^(99m)Tc-硫胶体后按摩乳腺对前哨淋巴结显像的影响。方法 回顾性分析2018年1月—2023年12月在浙江大学医学院附属邵逸夫医院核医学科接受前哨淋巴结显像且经病理证实为乳腺恶性肿瘤的119例女性患者的临床资料与显像结果。评估注射后进行乳腺按摩(注射+按摩组)与单纯注射(对照组)两组患者的基线指标、前哨淋巴结显像成功率、首颗前哨淋巴结显像时间及累计显像前哨淋巴结数量。采用χ^(2)检验比较两种注射方法组前哨淋巴结显像的显像成功率,采用Mann-Whitney U检验比较两组首颗前哨淋巴结显像时间及前哨淋巴结颗数。结果 两组患者在年龄、乳腺手术史、注射位置及注射点数方面,差异无统计学意义(P>0.05)。注射+按摩组的前哨淋巴结显像成功率显著高于单纯注射组(100.0% vs.86.1%,P=0.004)。注射+按摩组首颗前哨淋巴结显现时间显著短于单纯注射组(P=0.001)。此外,注射+按摩组显示多个前哨淋巴结的比率显著高于单纯注射组(P=0.001)。结论 在乳腺癌患者进行^(99m)Tc-硫胶体前哨淋巴结显像时,注射后进行局部乳腺按摩可提高显像成功率,缩短显像时间,并显示更多前哨淋巴结,从而提供更好的影像质量和临床决策支持。Objective This study evaluates the impact of breast massage following ~(99m)Tc-sulfur colloid injection on sentinel lymph node (SLN) imaging in breast cancer patients.Methods A retrospective analysis of 119 female patients with malignant breast tumors who underwent SLN imaging between January 2018 and December 2023 was performed.Patients were divided into two groups:one receiving breast massage after injection (injection + massage group) and the other receiving the injection alone (control group).Imaging success rate,time to first sentinel lymph node visualization,and the total number of visualized sentinel lymph nodes were compared.Results No significant differences in baseline characteristics were found between the two groups.The injection + massage group had a higher imaging success rate (100% vs.86.1%,P=0.004),a shorter median time to first visualization (P=0.001),and a higher rate of multiple sentinel lymph nodes visualized (P=0.001) compared to the injection-only group.Conclusion Breast massage following ~(99m)Tc-sulfur colloid injection enhances imaging success,reduces visualization time,and increases the number of sentinel lymph nodes visualized in breast cancer patients,potentially improving image quality and clinical decision-making.
