摘 要:目的:探讨炎性指标联合循环肿瘤细胞对乳腺癌新辅助化疗疗效的预测价值。方法:回顾性分析2020年1月至2022年12月在我院肿瘤内科就诊并接受新辅助化疗的125例乳腺癌患者的临床资料,根据治疗效果将患者分为pCR组39例和非pCR组86例。采用Logistic逐步回归分析乳腺癌NAC疗效的独立影响因素,采用受试者工作特征曲线(ROC)分析炎性指标联合循环肿瘤细胞对乳腺癌新辅助化疗疗效的预测价值。结果:单因素分析显示HR状态、Her-2状态、CTC状态、LMR、PLR、NLR、SII、SIRI与pCR相关(P<0.05)。两组的年龄、BMI、绝经情况、T分期、N分期、Ki-67比较差异均无统计学意义(P>0.05)。多因素分析显示Her-2状态、CTC状态、LMR、NLR、PLR、SII是影响乳腺癌新辅助化疗疗效的独立相关因素(P<0.05)。LMR、NLR、PLR、SII、Her-2状态、CTC状态、联合预测模型Pre的AUC分别为0.665、0.643、0.648、0.718、0.610、0.669、0.823。结论:LMR、NLR、PLR、SII等炎性指标联合CTC状态对乳腺癌新辅助化疗疗效具有较好的预测价值。Objective:To explore the predictive value of inflammatory markers combined with circulating tumor cells on the efficacy of neoadjuvant chemotherapy for breast cancer.Methods:The clinical data of 125 breast cancer patients who received neoadjuvant chemotherapy in the Department of Medical Oncology of our hospital from January 2020 to December 2022 were retrospectively analyzed,and the patients were divided into 39 cases in the pCR group and 86 cases in the non-pCR group according to the treatment effect.Logistic stepwise regression was used to analyze the independent influencing factors of NAC efficacy in breast cancer,and receiver operating characteristic curve(ROC)was used to analyze the predictive value of inflammatory markers combined with circulating tumor cells on the efficacy of neoadjuvant chemotherapy in breast cancer.Results:Univariate analysis showed that HR status,Her-2 status,CTC status,LMR,PLR,NLR,SII,and SIRI were correlated with pCR(P<0.05).There were no significant differences in age,BMI,menopause,T stage,N stage,and Ki-67 between the two groups(P>0.05).Multivariate analysis showed that Her-2 status,CTC status,LMR,NLR,PLR,and SII were independent correlated factors affecting the efficacy of neoadjuvant chemotherapy for breast cancer(P<0.05).The AUCs of LMR,NLR,PLR,SII,Her-2,CTC and joint prediction model Pre were 0.665,0.643,0.648,0.718,0.610,0.669 and 0.823,respectively.Conclusion:Inflammatory indicators such as LMR,NLR,PLR,SII and CTC status have good predictive value for the efficacy of neoadjuvant chemotherapy for breast cancer.
