摘　　要：肥胖(obesity)与肺癌(lung cancer, LC)是引起全球关注的重要公共健康问题。尽管肥胖已被确定为食管癌、乳腺癌、结肠癌等多种癌症的危险因素,但肥胖与肺癌的关系一直存在争议。此前多数研究认为,肥胖与肺癌并无相关性,甚至具有一定的保护作用。这种悖论现象的产生可能受吸烟、肥胖伴随药物的使用、肥胖测量方式、免疫紊乱等多种混杂因素的影响。主要由于身体质量指数(body mass index, BMI)测量肥胖具有局限性,它不能准确描述体内脂肪分布,不能区分脂肪与肌肉含量,掩盖了肥胖对肺癌患者的潜在影响。当将肥胖测量为内脏肥胖时,这种危害就变得明显。尤其是内脏肥胖与低BMI同时发生时,肺癌危险性显著增加,有助于识别肺癌的高风险人群。在临床工作中,选择准确的肥胖测量方法,及时对患者的健康状况进行评估,也为肺癌的预防与治疗提供新方向。该文旨在分析导致肥胖悖论的因素及其对肺癌的影响,进一步阐述肥胖与肺癌的相关性。Obesity and lung cancer are important public health issues that have attracted global attention.Although obesity has been identified as an important risk factor for various cancers such as esophageal cancer,breast cancer,and colon cancer,the relationship between obesity and lung cancer remains unclear.Most previous studies have suggested that obesity is not associated with lung cancer,and even has a certain protective effect.Obesity is associated with the use of drugs,smoking,obesity measurement methods,immune disorders and other confounding factors,resulting in this paradox.Due to the limitations of body mass index(BMI)in measuring obesity,it cannot accurately describe the distribution of body fat and cannot distinguish between fat and muscle content,which masks the potential impact of obesity on lung cancer patients.This hazard becomes apparent when obesity is measured as visceral obesity.In particular,the risk of lung cancer increased significantly when visceral obesity combined with low BMI,which is helpful to identify the high risk population of lung cancer.In clinical work,the selection of accurate obesity measurement methods and the timely assessment of the health status of patients will also provide a new direction for the prevention and treatment of lung cancer.The aim of this article is to analyze the factors leading to obesity paradox and its impact on lung cancer,and to further elaborate the correlation between obesity and lung cancer.
