头针联合口肌训练治疗儿童孤独症的效果及对患儿语言交流障碍、摄食行为的影响

摘　　要：目的观察头针联合口肌训练治疗儿童孤独症(CA)的临床疗效及对患儿语言交流障碍、摄食行为的影响。方法选取2022年5月至2023年3月于浙江中医药大学附属第三医院儿童康复科治疗的CA患儿78例,按照随机数字表法分为治疗组和对照组,各39例。对照组予口肌训练,治疗组在对照组治疗方法的基础上联合头针治疗。2组均治疗4个月后统计疗效。结果治疗组的总有效率为92.31%(36/39),显著高于对照组的71.79%(28/39),2组比较,差异有统计学意义(P<0.05)。治疗前2组的口部运动功能评分及摄食行为评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组的口部运动功能评分均显著升高,摄食行为评分均显著降低,与同组治疗前比较,差异均有统计学意义(P<0.05),且治疗组升高或降低更显著(P<0.05)。治疗前2组的行为表现及功能发展评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组上述评分均较同组治疗前显著升高,差异有统计学意义(P<0.05),且治疗组升高更显著(P<0.05)。治疗前2组的生活质量评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组生活质量评分均较同组治疗前显著升高,差异有统计学意义(P<0.05),且治疗组升高更显著(P<0.05)。结论头针联合口肌训练治疗儿童CA,可显著改善患儿语言表达及理解能力,纠正语言交流障碍,提高沟通及行为能力,有效改善摄食障碍问题,提高生活质量,且疗效显著,值得临床推广应用。Objective To observe the clinical efficacy of scalp acupuncture combined with oral muscle training in the treatment of childhood autism(CA)and its impact on language communication disorders and feeding behavior in children.Methods78 children with CA admitted to Child Rehabilitation Department of No.3 Affiliated Hospital of Zhejiang Chinese Medical University from May 2022 to March 2023 were randomly divided into a treatment group and a control group,with 39 cases in each group.The control group received oral muscle training,while the treatment group received scalp acupuncture combined with oral muscle training.After 4 months of continuous treatment,the clinical efficacy of both groups was evaluated.ResultsAfter treatment,the total effective rate of the treatment group 92.31%(36/39)was significantly higher than that of the control group 71.79%(28/39)(P<0.05);Before treatment,there was no significant difference in the scores of oral motor function and feeding behavior between the two groups(P>0.05).After treatment,the scores of oral motor function in the two groups were significantly improved(P<0.05),and the treatment group was higher than the control group(P<0.05);After treatment,the feeding behavior scores of the two groups significantly decreased(P<0.05),and the treatment group was lower than the control group(P<0.05);Before treatment,there was no significant difference in the PEP-3 score and PedsQL4.0 score between the two groups(P>0.05).After treatment,the above scores in the two groups were significantly improved(P<0.05),and the treatment group was higher than the control group(P<0.05).ConclusionThe combination of scalp acupuncture and oral muscle training has a definite therapeutic effect on children with CA,which can significantly enhance their oral motor function,correct their language communication disorders,enhance their communicative and behavinal skills improve their feeding behavior,and improve their quality of life.It's worthy of olinical promotion and application with its remarkable curative effect.
