改良印章法充填修复技术在后牙龋病患者中的应用效果

摘　　要：目的探究改良印章法充填修复技术在后牙龋病患者中的应用效果。方法选取2019年6月至2023年6月于医院牙体牙髓病科就诊的120例后牙深龋患者为研究对象,根据随机数字表法分为两组,每组60例。对照组采用传统合面形态堆塑进行咬合面形态充填,试验组采用改良印章法充填修复技术进行咬合面形态充填。比较两组后牙充填操作时间、充填完成后达到平衡所需的调整咬合次数、患者及未参与治疗医师对充填前后形态恢复情况的满意度[采用形态学视觉评估指数(VISA)评估]、充填体修复效果[采用改良后美国公共卫生署(US-PHS)充填体修复效果评价标准评估]。结果试验组充填操作时间长于对照组,充填完成后达到平衡所需的调整咬合次数少于对照组,差异有统计学意义(P<0.05);试验组患者及未参与治疗医师的VISA满意度高于对照组,差异有统计学意义(P<0.05);术后1年,两组US-PHS充填体修复效果比较,差异无统计学意义(P>0.05)。结论改良印章法充填修复技术可以减少充填完成后达到平衡所需的调整咬合次数,提高患者对充填后患牙形态的满意度,使牙面形态更逼真,且不影响术后充填体的修复效果。Objective To investigate the effect of the modified seal method of filling and restoration technique in patients with posterior caries.Methods 120 patients with deep caries in posterior teeth who attended the endodontics department of the hospital from June 2019 to June 2023 were selected for the study,and were divided into two groups of 60 cases each according to the random number table method.The control group was filled with the traditional conformal morphology stacking for conformal morphology filling,and the experimental group was filled with the modified seal method filling and restoration technique for conformal morphology filling.The two groups were compared in terms of filling operation time,the number of occlusal adjustments required to achieve equilibrium after completion of filling,the satisfaction of patients and non-participating practitioners with the restoration of morphology before and after filling[assessed using the visual index for shape analysis(VISA)],and the effect of filling restoration[assessed using United States Public Health Service(US-PHS)criteria].Results The operation time and the number of occlusal adjustments required to achieve equilibrium after the completion of filling in the test group was less than that in the control group,and the difference was statistically significant(P<0.05);the VISA satisfaction of patients in the test group and the non-participating physicians was higher than that in the control group,and the difference was statistically significant(P<0.05);1 year after the operation,the results of the repair of the US-PHS filling were compared between the two groups,and the difference was not statistically significant(P>0.05).Conclusion The modified seal method of filling and restoration technique can reduce the number of adjustments required to achieve equilibrium after filling completion,improve patient satisfaction with the morphology of the affected teeth after filling,and result in a more lifelike tooth surface morphology without compromising the restorative effe
