乌拉地尔和尼卡地平治疗脑出血患者高血压的效果及安全性系统评价

摘　　要：目的系统评价尼卡地平和乌拉地尔治疗脑出血(ICH)患者高血压的效果和安全性。方法系统检索并纳入维普网、万方数据知识服务平台、中国知网、PubMed、Embase等数据库建库至2024年7月有关尼卡地平与乌拉地尔治疗ICH患者高血压的临床研究,采用Stata软件进行Meta分析。结果本研究共纳入3篇文献,共265例ICH患者,其中尼卡地平组(治疗组)136例,乌拉地尔组(对照组)129例。Meta分析结果显示,在临床疗效方面,尼卡地平组的目标血压到达时间和血压变异系数均优于乌拉地尔(P<0.05);在安全性方面,尼卡地平组的不良反应发生率低于乌拉地尔组(P<0.05)。对心动过缓和心动过速等严重不良反应进行亚组分析,结果显示尼卡地平组的心动过缓发生率低于乌拉地尔组,心动过速发生率高于乌拉地尔组,但差异均无统计学意义(P>0.05)。结论尼卡地平在ICH患者的降压疗效和安全性方面均优于乌拉地尔,但心动过速发生率有高于乌拉地尔的趋势,仍需更多的临床研究数据进行验证。Objective To systematically evaluate the efficacy and safety of Nicardipine and Urapidil in the treatment of intracranial hemorrhage(ICH)patients with hypertension.Methods The clinical studies on Nicardipine and Urapidil in the treatment of ICH patients with hypertension were systematically searched and included in the databases such as VIP database,Wanfang Data Knowledge Service Platform,CNKI,PubMed and Embase from the establishment of the database to July 2024.Meta-analysis was performed by using Stata software.Results A total of 265 ICH patients were included in 3 literatures,which included 136 cases in Nicardipine group(treatment group)and 129 cases in Urapidil group(control group).Meta-analysis showed that the target blood pressure arrival time and coefficient of blood pressure variation in Nicardipine group were better than those in Urapidil group in terms of clinical efficacy(P<0.05);the incidence of adverse reactions in Nicardipine group was lower than that in Urapidil group in terms of safety(P<0.05);subgroup analysis of severe adverse reactions such as bradycardia and tachycardia showed that the incidence of bradycardia in Nicardipine group was lower than that in Urapidil group,while the incidence of tachycardia in Nicardipine group was higher than that in Urapidil group,but the differences were not statistically significant(P>0.05).Conclusions Nicardipine is better than Urapidil in terms of antihypertensive efficacy and safety in ICH patients with hypertension,but the incidence of tachycardia is higher than that of Urapidil,which needs more clinical study data to verify.
