十二指肠乳头肿物内镜切除临床效果分析

摘　　要：目的通过回顾性对比分析十二指肠乳头肿物内镜切除的临床效果。方法回顾分析2020年3月—2023年12月本院收治并符合纳入/排除标准的经内镜治疗和外科手术24位病人临床资料。从病变完整切除率、胆管/胰管插管成功率、住院费用、手术时长、住院天数、置管天数、术后并发症、术后病理结果来对比分析内镜下十二指肠乳头肿物切除术安全性和有效性。结果手术组患者术后感染、术后并发症发生率、住院费用、手术时长、术后住院天数、术后引流天数均高于内镜组(P<0.05)。内镜组完整切除率100%,胆/胰管支架留置成功率为91.7%。结论内镜下十二指肠乳头肿物切除术安全、有效,具有创伤小,不改变肠道及胆、胰解剖结构,术后康复快的优势。Objective To analyze the clinical outcomes of endoscopic resection of duodenal papillary masses by retrospective comparison.Methods The clinical data of 24 patients admitted to our hospital and meeting the inclusion/exclusion criteria for endoscopic treatment and surgery from March 2020 to December 2023 were retrospectively analyzed.The safety and efficacy of endoscopic duodenal papillary mass resection were compared and analyzed in terms of complete lesion resection rate,successful bile duct/pancreatic duct intubation rate,hospitalization cost,operative duration,hospitalization days,tube placement days,postoperative complications,and postoperative pathology results.Results Patients in the surgical group had a higher incidence of hospitalization cost,duration of surgery,postoperative hospitalization days,postoperative drainage days,postoperative infection,and postoperative complications than those in the endoscopic group(P<0.05).The complete resection rate in the endoscopic group was 100%,and the success rate of biliary/pancreatic stent retention was 91.7%.Conclusion Endoscopic duodenal papillary mass resection is safe and effective,with the advantages of minimally invasive,no change of intestinal and biliary/pancreatic anatomy,and rapid postoperative recovery.
