脑膜瘤侵犯颅骨9例临床病理分析

摘　　要：目的探讨脑膜瘤侵犯颅骨的临床病理特点。方法回顾性分析9例经手术治疗的脑膜瘤病人的临床病理资料。(原发于脑实质5例、原发于颅骨内2例,原发于眼球1例,原发于脊髓1例)。结果本研究包含男性患者5例,女性患者4例,中位年龄56.89岁(范围47-74岁)。9例患者的原发肿瘤部位不同,其中5例单发或合并发生于脑实质内,如额颞顶叶、额顶叶、枕叶等,2例起源于骨内,另1例原发于眼部,1例起源于颈椎管内。其中7例患者经治疗后均生存情况良好,无复发及并发症发生。结论脑膜瘤侵犯颅骨的病例较为少见,尤其是原发于颅骨内及眼球的病例罕见报道。本文旨在为侵袭颅骨或原发性骨内脑膜瘤的临床特征、治疗、预后及组织病理学特征提供一些线索。Objective To investigate the clinicopathological characteristics of meningiomas invading the skull.Methods Retrospective analysis of the clinicopathological data of 9 patients with meningiomas treated by surgery.5 cases originated in the brain parenchyma,2 cases originated in the skull,1 case originated in the eyeball,and 1 case originated in the spinal cord.Results This study included 5 male patients and 4 female patients with a median age of 56.89 years(range 47-74 years).The primary tumor sites of the 9 patients were different,of which 5 cases were isolated or combined in the brain parenchyma,such as frontotemporo-parietal lobe,fronto-parietal lobe,occipital lobe,etc.2 cases originated in bone,another case originated in the eye,and 1 case originated in the cervical vertebra.Among them,7 patients survived well after treatment without recurrence or complications.Conclusion The cases of meningiomas invading the skull are relatively rare,especially the cases that originated in the skull and the eyeballs are rarely reported.In this article,we hope to provide some clues on clinical characteristics,treatment,outcome of patients and histopathology features of meningiomas invading the skull or primary intraosseous.
