摘　　要：目的 探讨内镜黏膜下剥离(ESD)与外科手术对早期胃癌患者疗效、手术时间与并发症的影响。方法 回顾性分析本院收治的78例早期胃癌患者临床资料,以2021年1月为最早入组时间,以2024年12月为最晚入组时间。根据治疗方式的不同进行分组,将其中39例接受外科手术的患者设为外科手术组,39例采取ESD手术治疗的患者归为内镜组。观察两组疗效,统计其手术时间及术后恢复情况,并对比其在并发症方面的发生情况。结果 两组患者在治愈性切除率、完整切除率以及术后复发率方面比较,均未展现出明显差异(P>0.05);与外科手术组相比,内镜组在手术时间、术后首次排气时间、术后首次排便时间及住院时间方面均更短(P<0.05);内镜组术后并发症总发生率比外科手术组更低(P<0.05)。结论 两种手术方法均能够在早期胃癌患者中发挥较好的疗效,但ESD手术耗时更短,对于促进患者术后早期恢复和减少术后并发症更具优势。Objective To investigate the effect of endoscopic submucosal dissection(ESD)and surgery on the efficacy,operation time and complications of patients with early gastric cancer.Methods The clinical data of 78 patients with early gastric cancer admitted to our hospital were retrospectively analyzed.January 2021 was the earliest enrollment time,and December 2024 was the latest enrollment time.According to the different treatment methods,39 patients who underwent surgery were divided into the surgery group,and 39 patients who underwent ESD were divided into the endoscopic group.The curative effect of the two groups was observed,the operation time and postoperative recovery were counted,and the incidence of complications was compared.Results There was no significant difference in curative resection rate,en bloc resection rate and postoperative recurrence rate between the two groups(P>0.05).The operation time,postoperative first flatus time,postoperative first defecation time and hospital stay in the endoscopic group were shorter than those in the surgery group(P<0.05).The total incidence of postoperative complications in the endoscopic group was lower than that in the surgery group(P<0.05).Conclusions Both surgical methods can play a good effect in patients with EGC,but ESD is more advantageous in promoting early postoperative recovery and reducing postoperative complications because of its shorter operation time.
