摘　　要：目的 探究CEA、AFP、CA199和CA724在老年结直肠癌患者血清内的表达及意义。方法 选取2020年1月-2024年8月在湖南省人民医院进行治疗的66例老年结直肠癌患者作为研究组,另选取同期在本院进行体检的58例老年健康志愿者作为对照组,比较两组研究对象癌胚抗原(CEA)、甲胎蛋白(AFP)、糖链抗原199(CA199)、CA724,并按不同分期和癌症转移情况将老年结直肠癌患者分组,对组间CEA、AFP、CA199和CA724进行对比,此外,比较单一指标与联合检测的诊断效能。结果 研究组患者术前CEA、CA199和CA724水平均显著高于对照组(P<0.05),Ⅲ-Ⅳ期结直肠癌患者CEA、CA199水平均显著高于Ⅰ-Ⅱ期患者(P<0.05)。4种肿瘤标志物单独检测时,CA199的AUC最大,与其他指标相比,差异具有统计学意义(P<0.05)。联合检测与CEA、CA724、AFP单独检测相比,AUC较大,差异具有统计学意义(P<0.05),与CA199单独检测AUC相比,差异无统计学意义(P>0.05)。结论 老年结直肠癌患者CEA、AFP、CA199和CA724水平与健康人群相比有明显差异,通过对其进行早期检查,能够反映患者病情进展情况,且开展联合检测能够提高对结直肠癌转移的诊断准确率。Objective To investigate the expression and significance of CEA,AFP,CA199,and CA724 in the serum of elderly patients with colorectal cancer.Methods 66 elderly patients with colorectal cancer who received treatment at Hunan Provincial People's Hospital from January 2020 to August 2024 were selected as the study group,and 58 elderly healthy volunteers who underwent physical examinations at our hospital during the same period were selected as the control group.The two groups of study subjects were compared for carcinoembryonic antigen(CEA),alpha fetoprotein(AFP),carbohydrate antigen 199(CA199),and CA724.Elderly colorectal cancer patients were grouped according to different stages and cancer metastasis status,and CEA,AFP,CA199,and CA724 were compared between groups.In addition,the diagnostic efficacy of single index and combined detection was compared.Results The preoperative levels of CEA,CA199,and CA724 in the study group were significantly higher than those in the control group(P<0.05).The levels of CEA and CA199 in stage III-IV colorectal cancer patients were significantly higher than those in stage I-II patients(P<0.05).When the four tumor markers were detected separately,the AUC of CA199 was the highest,and the difference was statistically significant compared with other indicators(P<0.05).Compared with the individual detection of CEA,CA724,and AFP,the AUC of the combined detection was larger and the difference was statistically significant(P<0.05).However,compared with the individual detection of CA199,the AUC of the combined detection the difference was not statistically significant(P>0.05).Conclusion The levels of CEA,AFP,CA199,and CA724 in elderly patients with colorectal cancer are significantly different from those in healthy individuals.Early detection can reflect the development of the patient's condition,and combined examination can improve the diagnostic accuracy of colorectal cancer metastasis.
