摘　　要：目的分析iRoot BP Plus结合显微根尖手术治疗难治性根尖周炎的临床效果。方法选取2022年6月至2024年6月我院收治的难治性根尖周炎患者76例,随机分为观察组和对照组,各38例。两组均行显微根尖手术治疗,对照组采用银汞合金充填,观察组采用iRoot BP Plus填充。对比两组的临床疗效、肿痛情况、旧根尖周指数(O-PAI)、咀嚼功能(咀嚼效率、咬合力)、龈沟液炎性因子[白细胞介素-1(IL-1)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]水平、治疗满意度。结果两组的治疗总有效率比较无显著差异(P>0.05)。观察组肿痛率为5.26%,低于对照组的23.68%(P<0.05)。术后3个、6个月,观察组O-PAI均低于对照组(P<0.05)。术后3个月,观察组咀嚼效率、咬合力高于对照组,IL-1、IL-8、TNF-α水平低于对照组(P<0.05)。观察组后期指导、医疗环境、康复情况、医疗服务评分均高于对照组(P<0.05)。结论iRoot BP Plus结合显微根尖手术治疗难治性根尖周炎患者的效果显著,可减轻肿痛,促进根尖周组织恢复,改善咀嚼功能,减轻龈沟液炎症症状,提高治疗满意度。Objective To analyze the clinical effect of i Root BP Plus combined with microsurgical surgery in the treatment of refractory periapical radiculitis.Methods 76 patients with refractory periapical radiculitis admitted to our hospital from June 2022 to June2024 were selected and randomly divided into observation group and control group,with 38 cases in each group.All patients underwent microsurgical surgery,with the control group filled with silver amalgam and the observation group filled with i Root BP Plus.The clinical efficacy,swelling and pain status,old periapical index(O-PAI),chewing function(chewing efficiency,bite force),gingival crevicular fluid inflammatory factors[interleukin-1(IL-1),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)]levels,and treatment satisfaction were compared between the two groups.Results No significant difference was found in the total effective rate of treatment between the two groups(P>0.05).The swelling and pain rate of the observation group was 5.26%lower than 23.68%of the control group(P<0.05).At 3 and6 months after surgery,the O-PAI of the observation group was lower than that of the control group(P<0.05).3 months after surgery,the chewing efficiency and bite force of the observation group were higher than those of the control group,while the IL-1,IL-8,and TNF-αlevels were lower than those of the control group(P<0.05).The scores of post guidance,medical environment,rehabilitation status,and medical service in the observation group were higher than those in the control group(P<0.05).Conclusions IRoot BP Plus combined with microsurgical surgery has significant effect in the treatment of patients with refractory periapical radiculitis,can reduce the swelling and pain,promotie the periapical tissue recovery,improve the chewing function,alleviate the gingival crevicular fluid inflammation symptoms,and improve the treatment satisfaction.
