摘　　要：目的探讨以家庭医生签约制为指导的健康教育对社区脑卒中患者自我感受负担及心理状态的影响。方法将176例脑卒中恢复期患者随机分为两组,对照组(n=88)给予常规康复干预,观察组(n=88)给予常规康复干预+以家庭医生签约制为指导的健康教育,于干预前及干预后4周比较临床疗效。结果①干预后,观察组患者自我感受负担评分均较对照组显著下降(t=6.925、4.224、6.379,P均=0.000);②干预后,观察组患者S-AI和T-AI评分较对照组显著下降(t=8.333、7.539,P均=0.000),GSES评分显著升高(t=12.103,P=0.000);③干预后,两组患者FMA和BI评分比较,差异显著(t=17.116、12.643,P均=0.000)。结论采用以家庭医生签约制为指导的健康教育可显著减轻脑卒中患者自我感受负担,改善心理状态,促进患者功能恢复。Objective To explore the influence of health education guided by family doctor contract system on self-perceived burden and psychological status of stroke patients in a community.Methods 176 stroke patients were divided into two groups.The control group(n=88)was treated with routine rehabilitation intervention program,while the observation group(n=88)was treated with health education guided by family doctor contract system.After 4 weeks,two groups were analyzed.Results①Compared with control group,the scores of self-perceived burden of observation group was lower after treatment(t=6.925,4.224,6.379,P all=0.000).②Compared with control group,the scores of S-AI and T-AI of observation group were lower,while the scores of GSES was higher(t=8.333,7.539,12.103,P all=0.000).③The scores of FMA and BI of two groups were statistically significant(t=17.116,12.643,P all=0.000).Conclusion The health education guided by family doctor contract system can reduce self-perceived burden of patients with stroke,improve the psychological status and their limb function.
