摘 要:系统性红斑狼疮(Systemic lupus erythematosus,SLE)是一种以免疫复合物沉积为特征的结缔组织疾病,常伴有心脏损伤。常规超声心动图的主要参数如左心室射血分数(Left ventricular ejection fraction,LVEF)在亚临床左心室功能受损的诊断上敏感性不足。在LVEF保留的SLE患者中识别早期心功能的改变具有挑战性。目前,各种超声心动图技术如斑点追踪成像、实时三维超声心动图、无创心肌做功的广泛应用,能够更全面、更系统地评估SLE患者左心室功能改变,为临床早期识别SLE患者左心室受累情况提供依据。本文就SLE患者心脏损害的发病机制及超声心动图评估SLE患者左心室结构和功能变化的进展进行综述。Systemic lupus erythematosus(SLE)is a kind of connective tissue disease characterized by the deposition of immune complexes,usually accompanied by cardiac damage.The main parameters of conventional echocardiography,such as left ventricular ejection fraction(LVEF),have insufficient sensitivity in diagnosis of subclinical left ventricular dysfunctions.It is challenging to identify cardiac dysfunctions in SLE patients with a condition of preserved LVEF at an early stage.Nowadays,the extensive application of various echocardiography technology such as speckle tracking imaging,real-time three-dimensional echocardiography and myocardial work can evaluate the changes of left ventricular function in SLE patients more comprehensively and more systematically,providing the basis for early clinical identification of left ventricular involvement in SLE patients.This article reviews the progress of cardiac damage pathogenesis and echocardiography in evaluating the changes of left ventricular structure and function in SLE patients.
