摘　　要：目的探讨头痛宁胶囊联合佐米曲普坦片治疗偏头痛患者的疗效,并分析其对血液流变学、血清相关细胞因子水平的影响。方法选取2022年6月至2024年6月于信阳市第四人民医院诊治的98例偏头痛患者进行回顾性分析,依据治疗方案分为对照组(n=49)、研究组(n=49)。对照组予以佐米曲普坦片治疗,研究组予以头痛宁胶囊联合佐米曲普坦片治疗,连续治疗1个月。对比两组临床疗效,不良反应及治疗前后症状积分、发作次数、发作持续时间、疼痛程度[视觉模拟评分法(VAS)]。比较两组治疗前后脑血流动力学[颈内动脉(ICA)、大脑前动脉(ACA)、两侧椎动脉(VA)、基底动脉(BA)],血液流变学,血清相关细胞因子[5-羟色胺(5-HT)、神经肽Y(NPY)、P物质、一氧化氮(NO)、内皮素-1(ET-1)、降钙素基因相关肽(CGRP)]及血清炎性因子[肿瘤坏死因子-α(TNF-α)、细胞间黏附分子-1(ICAM-1)、环氧化酶2(COX-2)、同型半胱氨酸(Hcy)]水平。结果治疗后,研究组总有效率高于对照组(93.88%vs 77.55%,P<0.05);研究组症状积分、发作次数及VAS评分低于对照组,发作持续时间短于对照组(P<0.05);研究组ICA、ACA、VA、BA低于对照组(P<0.05);研究组血浆黏度、红细胞压积、红细胞聚集指数、红细胞变形指数低于对照组(P<0.05);研究组血清5-HT、NPY、P物质、NO、ET-1水平低于对照组,CGRP水平高于对照组(P<0.05);研究组血清TNF-α、ICAM-1、COX-2、Hcy水平低于对照组(P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论头痛宁胶囊联合佐米曲普坦片治疗偏头痛患者的疗效确切,不仅可改善临床症状、减轻疼痛程度,还可降低脑血流速度、改善血液流变学、抑制神经递质释放、保护血管内皮功能、抑制炎症反应,且具有一定安全性。Objective To investigate the therapeutic effect of toutongning capsules combined with zolmitriptan tablets in treating migraine patients,and analyze its effects on hemorheology and serum levels of related cytokines.Methods A retrospective analysis was conducted on 98 migraine patients who were treated in The Fourth People's Hospital of Xinyang from June 2022 to June 2024.They were divided into a control group(n=49)and a study group(n=49)according to the treatment plan.The control group was treated with zolmitriptan tablets,while the study group were treated with toutongning capsules combined with zolmitriptan tablets,continuous treatment for one month.The clinical efficacy and adverse reactions of the two groups were statistically compared,as well as symptom scores,attack frequency,attack duration,and pain severity[visual analog scale(VAS)]before and after treatment.The levels of cerebral hemodynamics[internal carotid artery(ICA),anterior cerebral artery(ACA),bilateral vertebral artery(VA),basilar artery(BA)],hemorheology,serum-related cytokines[5-hydroxytryptamine(5-HT),neuropeptide Y(NPY),substance P,nitric oxide(NO),endothelin-1(ET-1),calcitonin gene-related peptide(CGRP)],and inflammatory factors[tumor necrosis factor-α(TNF-α),intercellular adhesion molecule-1(ICAM-1),cyclooxygenase-2(COX-2),homocysteine(Hcy)]were compared between the two groups before and after treatment.Results After treatment,the total effective rate of the study group was higher than that of the control group(93.88%vs 77.55%,P<0.05);the symptom score,number of attacks,and VAS score in the study group were lower than those in the control group,and the duration of attacks was shorter than that in the control group(P<0.05);the ICA,ACA,VA,and BA in the study group were lower than those in the control group(P<0.05);the plasma viscosity,hematocrit,erythrocyte aggregation index,and erythrocyte deformation index in the study group were lower than those in the control group(P<0.05);the levels of serum 5-HT,NPY,substance P,NO,and ET-1 in the stud
