喹硫平联合丙戊酸钠对双相情感障碍患者BRMS评分及神经递质表达的影响  

Effects of Quetiapine Combined with Sodium Valproate on BRMS Scores and Neurotransmitter Expressions in Patients with Bipolar Disorder

摘　　要：目的探讨双相情感障碍患者应用喹硫平和丙戊酸钠治疗方案对贝克-拉范森躁狂量表(BRMS)评分及神经递质表达的影响。方法按随机数字表法将2023年6月至2024年6月于赣州市第三人民医院治疗的68例双相情感障碍患者分为两组,每组34例。对照组使用丙戊酸钠治疗,观察组在对照组基础上加用喹硫平治疗,两组均治疗2个月。比较两组神经递质[多巴胺(DA)、5-羟色胺(5-HT)、去甲肾上腺素(NE)]水平,BRMS评分,临床疗效和不良反应。结果相比于对照组,观察组治疗总有效率较高(P<0.05);观察组BRMS评分、NE、5-HT、DA水平均低于对照组(P<0.05);两组不良反应比较,差异无统计学意义(P>0.05)。结论相比于单用丙戊酸钠,联合喹硫平治疗双相情感障碍疗效确切,可有效缓解患者躁狂症状,降低患者神经递质水平,安全可靠,具有广泛应用前景。Objective Exploring the effects of quetiapine and sodium valproate on Bech-Rafaelsen Mania Rating Scale(BRMS)scores as well as the levels of neurotransmitter expressions in patients with bipolar disorder.Methods Participants of this study were 68 patients with bipolar disorder who were admitted to Ganzhou Third People's Hospital from June 2023 to June 2024.The enrolled patients were randomly divided into two groups using a random number table method,with 34 cases in each group.The control group received therapy with sodium valproate,while the observation group were provided with sodium valproate and quetiapine,both groups were treated for 2 months.Furthermore,inter-group comparison were conducted regarding the levels of neurotransmitters(DA,5-HT and NE),BRMS scores,clinical efficacy and adverse reactions.Results The observation group exhibited a higher overall response rate than that of the control group(P<0.05).The observation group showed lower BRMS scores as well as decreased levels of NE,5-HT and DA compared to the control group(all P<0.05).There was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion Compared to sodium valproate monotherapy,the use of quetiapine and sodium valproate in combination demonstrates significant efficacy in treating patients with bipolar disorder,which can effectively alleviate manic symptoms and reduce neurotransmitter levels,with reliable safety,the combined regimen has wide application prospect.
