镇肝熄风汤合温胆汤治疗阿尔茨海默病对精神行为症状及血清甲状腺激素的影响

摘　　要：目的:观察镇肝熄风汤合温胆汤治疗阿尔茨海默病(AD)对精神行为症状(BPSD)及血清甲状腺激素的影响。方法:将2022年1月—2023年1月衢州市第三医院收治的AD患者60例,通过随机数字表法分为对照组、试验组各30例。对照组采用常规抗痴呆药物治疗,试验组在常规抗痴呆药物基础上加用镇肝熄风汤合温胆汤治疗。比较2组BPSD指标[病理行为评分(BEHAVE-AD)、神经精神量表(NPI)、阳性症状量表(SAPS)]、血清甲状腺激素[三碘甲状腺原氨酸(T3)、甲状腺素(T4)]水平、不良反应和临床疗效。结果:试验组总有效率为93.33%(28/30),对照组为60.00%(18/30),2组比较,差异有统计学意义(P<0.05)。治疗后,2组BEHAVE-AD、NPI、SAPS评分均较治疗前降低(P<0.05),且试验组3项评分均低于对照组(P<0.05)。治疗后,2组T3、T4水平均较治疗前升高(P<0.05),且试验组T3、T4水平均高于对照组(P<0.05)。治疗期间,试验组不良反应发生率为16.67%(5/30),对照组为26.67%(8/30),2组比较,差异无统计学意义(P>0.05)。结论:镇肝熄风汤合温胆汤能够减轻AD患者的精神行为症状,改善血清甲状腺激素水平,提高临床疗效。Objective:To observe the effect of Zhengan Xifeng Decoction combined with Wendan Decoction on behavioral and psychological symptoms(BPSD)and serum thyroid hormones in the treatment of Alzheimer's disease(AD).Methods:A total of 60 AD patients treated at the Third Hospital of Quzhou from January 2022 to January 2023 were divided into the control group and the trial group according to the random number table method,with 30 cases in each group.The control group was treated with routine anti-dementia medicines,and the trial group was additionally treated with Zhengan Xifeng Decoction combined with Wendan Decoction in addition to routine antidementia medicines.The two groups were compared in terms of BPSD indicators[Behavioral Pathology in Alzheimer's Disease Rating Scale(BEHAVE-AD),Neuropsychiatric Inventory(NPI),Scale for the Assessment of Positive Symptoms(SAPS)],serum thyroid hormones[triiodothyronine(T3),thyroxine(T4)],adverse reactions,and clinical effects.Results:The total effective rate in the trial group was 93.33%(28/30),compared to 60.00%(18/30)in the control group,with a significant difference between the two groups(P<0.05).After treatment,the scores of BEHAVE-AD,NPI,and SAPS in the two groups were decreased when compared with those before treatment(P<0.05),and the scores in the trial group were lower than those in the control group(P<0.05).After treatment,the levels of T3 and T4 in the two groups were increased when compared with those before treatment(P<0.05),with the trial group showing higher levels than the control group(P<0.05).During the treatment period,the incidence of adverse reactions was 16.67%(5/30)in the trial group and 26.67%(8/30)in the control group,with no significant difference between the two groups(P>0.05).Conclusion:Zhengan Xifeng Decoction combined with Wendan Decoction can effectively reduce behavioral and psychological symptoms in AD patients,improve serum thyroid hormone levels and enhance clinical efficacy.
