摘  要:目的:通过筛选与子宫内膜癌(EC)预后相关的整合素家族基因,并根据最佳基因组合构建预后模型。方法:获取TCGA数据库中EC患者的转录组和临床数据。利用limma包筛选EC的差异表达基因,利用survminer包筛选EC预后相关基因。利用glmnet包提供的LASSO算法构建基于整合素家族基因的预后模型。利用Kaplan-Meier和ROC曲线评估预后模型的预测价值。利用多因素Cox回归分析对模型进行独立性分析。利用DOSE包、ClusterProfler包、GSVA包、GSEA软件包进行富集分析。利用TIDE算法预测免疫治疗应答效应。利用oncoPredict包进行药物反应预测。结果:构建由ITGA7、ITGAL、ITGA6、ITGB1BP2及ITGA10等整合素家族基因组成的EC预后模型。基于预后模型,高风险组的EC患者预后明显较低风险组患者更为不良,预后模型具备良好的预测效能且是EC的独立预后因素。高、低风险组患者在不同通路中富集。低风险组患者更可能从免疫治疗中获益,也更有可能从顺铂、阿糖胞苷、多西他赛、紫杉醇、他莫昔芬及长春瑞滨等药物治疗中获益。结论:基于整合素家族基因的预后模型是EC预后预测工具,有望指导患者的个性化治疗。Objective:To identify and analyze the integrin family genes associated with the prognosis of endometrial cancer(EC),and subsequently develop a prognostic model based on the optimal combination of these genes.Methods:The transcriptome and clinical data of EC patients were acquired from TCGA database.The limma package was used to identify differentially expressed genes,and the survminer package was employed for identifying prognostic genes.The LASSO algorithm provided by the glmnet package was utilized to construct a prognostic model based on genes from the integrin family.The prognostic model was evaluated for its predictive efficacy through Kaplan-Meier and ROC curves.The independence analysis was conducted using multivariate Cox regression analysis.The enrichment analysis was conducted using the DOSE,ClusterProfiler,GSVA,and GSEA packages.The TIDE algorithm was employed for the prediction of immunotherapy response.The oncoPredict package was utilized for drug response prediction.Results:The prognostic model for EC was established based on the optimal combination of integrin family genes including ITGA7,ITGAL,ITGA6,ITGB1BP2,and ITGA10.The prognostic model revealed a significantly worse prognosis for EC patients in the high-risk group compared to those in the low-risk group.The prognostic model demonstrated excellent predictive efficacy and served as an independent prognostic factor for EC.The patients with high-and low-risk demonstrated distinct enrichments in pathways.The patients with low-risk demonstrated a higher likelihood of benefiting from immunotherapy as well as the administration of cisplatin,cytarabine,docetaxel,paclitaxel,tamoxifen,and vinorelbine.Conclusion:The prognostic model utilizing integrin family genes serves as a valuable tool for predicting the prognosis of EC patients,thereby facilitating personalized treatment strategies.
