摘 要:目的:监测2023年银川市淋球菌对7种抗生素的耐药趋势及淋球菌多抗原基因分型,为临床有效选择抗生素提供实验室依据。方法:收集淋球菌临床分离菌株21株,采用琼脂稀释法检测其对7种常用抗生素的药物敏感性;基于全基因组测序技术预测耐药表型、分析多抗原测序型别,对比体外抗菌药物敏感性试验和耐药表型预测结果符合率。结果:21株菌株对青霉素和四环素的耐药率分别为80.96%和76.19%,环丙沙星耐药率为100.00%,其余均为0;青霉素、四环素、头孢克肟、大观霉素、阿奇霉素体外抗菌药物敏感性试验结果和耐药表型预测结果符合率均为100%,头孢曲松为80.95%;全基因测序后分析得到等位基因porB 11种、tbpB 7种,其中porB 64和tbpB 2565为占比最多的型别,多抗原测序型别共12种。结论:头孢曲松、大观霉素、阿奇霉素和头孢克肟仍可作为银川市淋病患者首选药物,但需要长期监测,加强对阿奇霉素、头孢曲松及头孢克肟低敏株的关注度;多抗原测序型别分析发现银川市新的基因型菌株较多。OBJECTIVE To monitor the resistance trend of Neisseria gonorrhoeae to 7 antibiotics and the genotyping characteristics of N.gonorrhoeae multi-antigen in Yinchuan in 2023,so as to provide laboratory basis for effective selection of antibiotics in clinic.METHODS A total of 21 strains of N.gonorrhoeae were collected,and their drug sensitivity to 7 common antibiotics was detected by agar dilution method.Based on genome-wide sequencing technology,to predict drug resistance phenotype and analyze NG-MAST type,and the coincidence rate between in vitro Antimicrobial Susceptibility Test(AST)and drug resistance phenotype prediction was compared to explore the genotype of N.gonorrhoeae in Yinchuan.RESULTS No isolate was resistant to CFM,CRO,AZM,SPT,PEN and TET were 80.96%and 76.19%.AST and drug resistance phenotype prediction of PEN,TET,CFM,SPT and AZM were all consistent with 100%,and CRO was 80.95%.After sequencing the whole gene,there were 11 and 7 alleles of porB and tbpB,respectively,among which porB 64 and tbpB 2565 accounted for the most.There are 12 types of NG-MAST.CONCLUSION CFM,CRO,AZM and SPT can still be used as the first choice drugs for gonorrhea patients in Yinchuan,but long-term monitoring is needed to strengthen the attention of AZM,CRO and CFM low-sensitive strains in the area.Typological analysis showed that there were many strains with new genotype of NG-MAST in Yinchuan.
