摘 要:目的分析胃癌患者癌组织微小核糖核酸(miR)-183-5p、哺乳动物雷帕霉素靶蛋白(mTOR)表达与肿瘤恶性生物学行为及预后的关系。方法收集经手术切除的94例胃癌患者的胃癌组织和癌旁组织标本(距离癌灶边缘距离>5 cm)。记录入组患者的病理资料,比较患者癌组织和癌旁组织miR-183-5p、m TOR表达量,分析癌组织miR-183-5p与mTOR表达的相关性。分析miR-183-5p、mTOR表达与胃癌临床病理特征之间的关系。出院后跟踪随访,随访记录两组患者生存情况,分析miR-183-5p、mTOR表达量与预后的关系。结果癌组织miR-183-5p、mTOR的相对表达量高于癌旁组织(t分别=14.71、11.11,P均<0.05)。Pearson分析显示,癌组织miR-183-5p与mTOR相对表达量呈正相关(r=0.21,P<0.05)。淋巴结转移、浸润深度超过深肌层、TNM分期为Ⅲ期和Ⅳ期、分化程度为低分化的胃癌患者miR-183-5p、mTOR相对表达量均明显高于无淋巴结转移、浸润深度累及深肌层、TNM分期为Ⅰ期和Ⅱ期、分化程度为中高分化的胃癌患者(t分别=8.53、8.19、12.96、5.00;14.45、15.57、14.62、16.38,P均<0.05)。随访期间共发生37例患者死亡,以miR-183-5p表达量平均值3.92为界,将患者分为高表达组(miR-183-5p≥3.92,n=39)和低表达组(miR-183-5P<3.92,n=55);以mTOR表达量平均值1.96为界,将患者分为高表达组(mTOR≥1.96,n=34)和低表达组(mTOR<1.96,n=60)。Kaplan-Meier分析显示,miR-183-5p高表达组总生存率、无病生存率均低于miR-183-5p低表达组(χ^(2)分别=14.68、24.63,P均<0.05),mTOR高表达组总生存率、无病生存率均低于mTOR低表达组(χ^(2)分别=15.42、23.51,P均<0.05)。结论胃癌患者癌组织miR-183-5p、mTOR表达与肿瘤恶性生物学行为和预后有关,随着胃癌组织淋巴结转移、浸润深度增加、临床分期增加、分化程度降低,miR-183-5p、mTOR表达越高,预后越差。Objective To analyze the relationship between micro RNA(miR)-183-5p,mammalian target of rapamycin(mTOR)and biological behaviors of tumor malignancy,prognosis in patients with gastric cancer.Methods The specimens of surgically excised gastric cancer tissues and para-carcinoma tissues(distance from the edge of cancer>5 cm)were collected from 94 patients with gastric cancer.The pathological data of enrolled patients were recorded.The expression levels of miR-183-5p and mTOR in cancer tissues and para-carcinoma tissues of patients were compared.The correlation between miR-183-5p and mTOR in cancer tissues,and the relationship between miR-183-5p,mTOR and clinicopathological characteristics of gastric cancer were analyzed.All were followed up after discharge to record survival.The relationship between miR-183-5p,mTOR and prognosis was analyzed.Results The relative expression levels of miR-183-5p and mTOR in cancer tissues were higher than those in para-carcinoma tissues(t=14.71,11.11,P<0.05).Pearson analysis showed that miR-183-5p was positively correlated with mTOR in cancer tissues(r=0.21,P<0.05).The relative expression levels of miR-183-5p and mTOR in patients with lymph node metastasis,invasion depth exceeding deep muscle layer,TNM staging at stageⅢ-Ⅳand low differentiation were significantly higher than those with non-metastasis,invasion depth involving deep muscle layer,TNM staging at stageⅠ-Ⅱand medium to high differentiation(t=8.53,8.19,12.96,5.00,14.45,15.57,14.62,16.38,P<0.05).There were 37 patients died during follow-up.According to mean miR-183-5p(3.92),patients were divided into high expression group(miR-183-5p≥3.92,n=39)and low expression group(miR-183-5p<3.92,n=55).According to mean mTOR(1.96),patients were divided into high expression group(mTOR≥1.96,n=34)and low expression group(mTOR<1.96,n=60).Kaplan-Meier analysis showed that overall survival rate and disease-free survival rate in high-expression miR-183-5p group were lower than those in low-expression miR-183-5p group(χ^(2)=14.68,24.63,
