帕利哌酮联合奥氮平治疗成人急性期精神分裂症患者疗效及对PANSS、PSP评分的影响

摘　　要：目的探讨帕利哌酮联合奥氮平治疗成人急性期精神分裂症患者疗效及对阳性和阴性症状量表(PANSS)、个人和社会功能量表(PSP)评分的影响。方法选取急性期精神分裂症患者100例为研究对象。按照随机数字表法分为观察组和对照组,各50例。观察组予以帕利哌酮针剂联合奥氮平片治疗,对照组予以奥氮平片治疗。比较两组治疗前后的PANSS评分、PSP评分、相关实验室指标水平及攻击行为情况,并进行安全性分析。结果治疗后,观察组患者的PANSS评分阳性和阴性症状、一般精神病理症状和改版外显攻击行为(MOAS)量表评分均低于对照组,PSP评分高于对照组,差异均有统计学意义(t分别=-3.16、-5.01、-4.34、-18.06、6.41,P均<0.05);治疗后,观察组患者的血清泌乳素、皮质醇、白介素-18、磷酸化雷帕霉素靶蛋白水平均低于对照组,差异均有统计学意义(t分别=-8.99、-7.74、-5.74、-5.77,P均<0.05)。两组不良反应发生率比较,差异无统计学意义(χ^(2)=1.03,P>0.05)。结论帕利哌酮联合奥氮平治疗成人急性期精神分裂症患者在改善症状、降低PANSS评分以及提高PSP评分方面都具有显著的疗效,有助于提高个人和社会功能,安全性良好。Objective To explore the curative effect of paliperidone combined with olanzapine on positive and negative symptom scale(PANSS)and personal and social functioning scale(PSP)in adult patients with acute schizophrenia.Methods A total of 100 patients with acute schizophrenia were selected as the study objects.According to random number table method,the patients were divided into observation group and control group,with 50 cases each.The observation group was treated with paliperidone injection combined with olanzapine tablet,and the control group was treated with olanzapine tablet.The PANSS score,PSP score,relevant laboratory index level and attack behavior of the two groups before and after treatment were compared,and security analysis was conducted.Results After treatment,the PANSS positive and negative symptoms,general psychopathological symptoms and modified explicit aggressive behavior(MOAS)scale scores in observation group were lower than those in control group,and PSP score was higher than that in control group,with statistical significance(t=-3.16,-5.01,-4.34,-18.06,6.41,P<0.05).After treatment,the levels of serum prolactin,cortisol,interleukin-18 and phosphorylated rapamycin target protein in observation group were lower than those in control group,with statistical significance(t=-8.99,-7.74,-5.74,-5.77,P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(χ^(2)=1.03,P>0.05).Conclusion Paliperidone combined with olanzapine in the treatment of adult patients with acute schizophrenia has significant efficacy in improving symptoms,reducing PANSS score and increasing PSP score.It is conducive to improving personal and social function with good safety.
