安宁疗法对老年肿瘤晚期患者癌痛及癌因性疲乏的效果分析

摘　　要：目的分析安宁疗法对老年肿瘤晚期患者癌痛及癌因性疲乏(cancer-related fatigue,CRF)的效果。方法回顾性纳入马鞍山市人民医院2021年1月至2023年12月收治的192例老年肿瘤晚期患者,采用简单随机抽样法分为观察组(n=102)和对照组(n=90)。所有患者均接受常规综合治疗和干预,观察组患者在此基础上加用安宁疗法干预。比较两组患者干预前后癌痛、CRF、生命质量、心理状态、总体幸福感和尊严感。结果干预1、2、3、4周后,观察组患者数字分级评分法评分均显著低于对照组(均P<0.05)。干预4周后,观察组患者修订版Piper疲乏量表各维度评分、患者欧洲癌症治疗研究组织癌症患者生命质量测定量表各维度评分和总分、焦虑症筛查量表评分、抑郁筛查量表评分和尊严量表评分均显著低于对照组(均P<0.05),但总体幸福感量表评分显著高于对照组(P<0.05)。结论安宁疗法能帮助老年肿瘤晚期患者减轻癌痛和CRF,且可提升患者生活质量、幸福感和尊严感,缓解负性情绪。Objective To analyze the effect of hospice therapy on cancer pain and cancer-related fatigue(CRF)in elderly patients with advanced cancer.Method A retrospective study was conducted on 192 elderly patients with advanced cancer who were admitted to Maanshan People's Hospital from January 2021 to December 2023.The patients were divided into observation group(n=102)and control group(n=90)using a simple random sampling method.All patients received conventional comprehensive treatment and intervention,while the observation group additionally received hospice therapy.The cancer pain,CRF,quality of life,psychological status,overall well-being,and sense of dignity before and after the intervention between the two groups were compared.Result After 1,2,3,and 4 weeks of intervention,the numerical rating scale scores of observation group were significantly lower than those of control group(all P<0.05).After 4 weeks of intervention,the scores of each dimension and the total score of the revised Piper fatigue scale the scores of each dimension and the total score of the European Organization for Reasearch and Treatment of cancer quality of life questionnaire core-30,the anxiety screening scale score,the depression screening scale score,and the dignity scale score in observation group were significantly lower than those in control group(all P<0.05).However,the overall well-being scale score in observation group was significantly higher than that in control group(P<0.05).Conclusion Hospice therapy can help elderly patients with advanced cancer alleviate cancer pain and CRF,while also improving their quality of life,sense of wellbeing,and dignity,as well as mitigating negative emotions.
