保乳手术联合前哨淋巴活检术治疗早期乳腺癌患者的效果观察

摘　　要：目的:探讨保乳手术联合前哨淋巴活检术在早期乳腺癌(EBC)患者中的应用效果。方法:回顾性分析2021年6月—2023年1月河南省林州市人民医院普外科收治的86例EBC患者的临床资料。按照手术方法的不同分为对照组(43例,行改良根治术)和观察组(43例,行保乳手术联合前哨淋巴活检术),两组患者均持续随访6个月。比较两组患者围术期指标、并发症发生情况、生活质量及乳房美观满意度。结果:与对照组相比,观察组患者手术时间较长,术中出血量、总引流量均较少,腋窝拔管及住院时间均较短,差异均有统计学意义(t=15.717、18.885、24.661、15.240、10.239,P<0.05);观察组患者并发症发生率低于对照组,差异有统计学意义(χ^(2)=4.074,P<0.05);护理后,观察组患者乳腺癌人群特异性问卷(QLQ-BR23)中的症状维度评分低于对照组,功能维度评分高于对照组,差异均有统计学意义(t=7.068、4.264,P<0.05);观察组患者乳房美观满意度高于对照组,差异有统计学意义(χ^(2)=5.460,P<0.05)。结论:针对EBC患者实施保乳手术与前哨淋巴活检术治疗,能够改善其围手术期指标,降低并发症发生风险,提高生活质量,且患者对乳房美观的满意度较高。Objective:To investigate the effect of breast conserving surgery combined with sentinel lymph node biopsy in early breast cancer(EBC)patients.Methods:A retrospective analysis was conducted on the clinical data of 86 patients with EBC admitted to the Department of General Surgery at Linzhou People’s Hospital from June 2021 to January 2023.They were divided into two groups according to different surgical methods.The control group(43 cases)underwent modified radical surgery,while the observation group(43 cases)underwent breast conserving surgery combined with sentinel lymph node biopsy.Both groups were followed up for 6 months.Perioperative indicators,incidence of complications,quality of life and satisfaction with breast aesthetics between two groups were compared.Results:Compared with the control group,the operation time of observation group was longer,the intraoperative blood loss and total drainage volume were less,and the axillary extubation and hospitalization time were shorter,with statistically significant difference(t=15.717,18.885,24.661,15.240,10.239;P<0.05).The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(χ^(2)=4.074,P<0.05).After nursing,the symptom dimension score of QLQ-BR23 in the observation group was lower than that in the control group,and the functional dimension score was higher than that in the control group,with statistically significant difference(t=7.068,4.264;P<0.05).The satisfaction of breast aesthetics in the observation group was higher than that in the control group,and the difference was statistically significant(χ^(2)=5.460,P<0.05).Conclusion:Implementing breast conserving surgery and sentinel lymph node biopsy for EBC patients can improve their perioperative indicators,reduce the risk of complications,improve their quality of life,and increase patient satisfaction with breast aesthetics.
