利用倾向性评分匹配探讨牙列缺失义齿种植术后慢性疼痛的影响因素

摘　　要：目的:利用倾向性评分匹配(PSM)探讨牙列缺失义齿种植术后慢性疼痛的影响因素。方法:回顾性分析2022年10月—2023年6月南阳医学高等专科学校第一附属医院收治的379例行义齿种植术的牙列缺失患者临床资料,根据术后是否发生慢性疼痛分为慢性疼痛组和非慢性疼痛组。运用PSM对慢性疼痛组和非慢性疼痛组患者进行1∶1匹配,采用logistic回归分析探讨牙列缺失义齿种植术后慢性疼痛的影响因素。结果:logistic回归分析结果显示,种植体数目、牙槽骨骨质Ⅰ类、合并糖尿病、吸烟史、口腔卫生习惯不良、骨增量手术、定期复查均是牙列缺失患者义齿种植术后发生慢性疼痛的影响因素(OR=1.714、1.660、1.990、1.603、1.806、1.481、0.523,P<0.05)。结论:利用PSM分析得出,牙列缺失患者义齿种植术后发生慢性疼痛的影响因素包括种植体数目、牙槽骨骨质Ⅰ类、合并糖尿病、吸烟史、口腔卫生习惯不良、骨增量手术、定期复查。Objective:To explore the influencing factors of chronic pain after denture implant with dentition missing by using propensity score matching(PSM).Methods:The clinical data of 379 patients with dentition missing who underwent denture implantation in the hospital from October 2022 to June 2023 were retrospectively analyzed.According to whether chronic pain occurred after surgery,they were divided into chronic pain group and non-chronic pain group,and the general data of the two groups were compared.PSM was used to match 1∶1 patients in the chronic pain group and the non-chronic pain group.Multivariate logistic regression analysis was used to explore the influencing factors of chronic pain after denture implantation with dentition missing.Results:Multivariate logistic regression analysis showed that the number of implants,alveolar bone typeⅠ,combined diabetes,smoking history,poor oral hygiene habits,bone increment surgery and regular reexamination were the influencing factors for chronic pain after denture implantation in patients with denture deletion(OR=1.714,1.660,1.990,1.603,1.806,1.481,0.523;P<0.05).Conclusion:PSM analysis shows that the influencing factors of chronic pain in patients with dentition missing after denture implantation include the number of implants,alveolar bone typeⅠ,diabetes,smoking history,poor oral hygiene habits,bone augmentation surgery and regular reexamination.
