HPV、TCT鉴别诊断宫颈癌及癌前病变的价值分析

摘　　要：目的探讨人乳头瘤病毒(HPV)、液基薄层细胞(TCT)诊断宫颈癌及癌前病变价值,为临床提供参考。方法选取2022年1月至2023年6月河南省信阳市平桥区妇幼保健院收治的200例疑似宫颈病变患者为研究对象,所有患者均行HPV、TCT、病理检查,以病理检查结果为金标准,比较HPV、TCT单独及联合的检测结果,分析两种检查单独及联合检查的一致性。结果200例疑似宫颈病变患者病理学检查结果显示阳性18例,阴性182例;HPV检测结果显示阳性54例,阴性146例;TCT检测结果阳性43例,阴性157例;HPV、TCT联合检测阳性31例,阴性169例。一致性分析结果显示,HPV、TCT联合检测诊断宫颈癌及癌前病变的Kappa=0.655(P<0.05),高于HPV、TCT单独检测(Kappa=0.165、0.343,P<0.05);HPV、TCT联合检测的敏感度为94.44%、特异度为92.31%、准确率为92.50%均高于单项检测(P<0.05)。结论HPV、TCT联合检测对宫颈癌及癌前病变检出率高,对宫颈癌早期筛查具有重要意义。Objective To explore the diagnostic value of human papillomavirus(HPV)and liquid-based cytology(TCT)in cervical cancer and precancerous lesions,providing a reference for clinical practice.Methods A total of 200 patients with suspected cervical lesions treated at Pingqiao District Maternal and Child Health Hospital in Xinyang,Henan,from January 2022 to June 2023 were selected for the study.All patients underwent HPV testing,TCT,and pathological examination,with pathological results serving as the gold standard.The results of HPV and TCT,both separately and combined,were compared,and the consistency of the two tests was analyzed.Results Among the 200 patients with suspected cervical lesions,the pathological examination showed 18 positive and 182 negative results.HPV testing yielded 54 positive and 146 negative results,while TCT results were 43 positive and 157 negative.The combined HPV and TCT testing showed 31 positive and 169 negative results.Consistency analysis indicated that the Kappa value for diagnosing cervical cancer and precancerous lesions using combined HPV and TCT testing was 0.655(P<0.05),which was higher than the Kappa values for HPV and TCT testing alone(0.165 and 0.343,P<0.05).The combined HPV and TCT testing had a sensitivity of 94.44%,specificity of 92.31%,and accuracy of 92.50%,all higher than those of individual tests(P<0.05).Conclusion Combined HPV and TCT testing demonstrates a high detection rate for cervical cancer and precancerous lesions,and is of significant importance for early screening of cervical cancer.
