摘 要:目的探讨振幅整合脑电图(amplitude integrated electroencephalogrphy,aEEG)在不同胎龄早产儿脑发育成熟度中的应用价值。方法选取2022年1月-2024年1月我院收治的160例早产儿,分成28-29+6周组、30-31+6周组、32-33+6周组、34-36+6周组,所有早产儿均出生后3d内及出生后2周内纠正胎龄28-36+6周内接受aEEG检查,分别从连续性、睡眠-觉醒周期、下边界振幅和带宽对图像进行评估。结果aEEG图像连续性电压及睡眠-觉醒周期出现率随胎龄增加逐渐升高;aEEG图像连续性、睡眠-觉醒周期、下边界振幅评分均呈上升趋势,而带宽逐渐变窄,各胎龄间存在显著差异(P<0.05)。随纠正胎龄增加,aEEG图像逐渐趋向成熟,30-33+6周明显高于同胎龄早产儿(P<0.05)。结论胎龄增大促使早产儿aEEG图像趋向成熟,宫外生活对早产儿脑成熟具有加速作用,aEEG在评估早产儿脑发育成熟度方面具有重要价值。Objective To investigate the application value of amplitude integrated electroencephalogrphy(aEEG)in brain maturity of premature infants of different gestational ages.Methods A total of 160 preterm infants admitted to our hospital from January 2022 to January 2024 were selected and divided into 28-29+6 weeks group,30-31+6 weeks group,32-33+6 weeks group,and 34-36+6 weeks group.All preterm infants underwent aEEG examination within 3 days after birth and 2 weeks after birth.The images were evaluated in terms of continuity,sleep-wake cycle,lower boundary amplitude,and bandwidth.Results The continuous voltage of aEEG image and the occurrence rate of sleep-wake cycle increased with gestational age.The score of aEEG image continuity,sleep-wake cycle and lower boundary amplitude increased,while the bandwidth narrowed gradually,and there were significant differences among gestational ages(P<0.05).With the increase of corrected gestational age,aEEG images tended to mature gradually,and were significantly higher at 30-33+6 weeks than those at the same gestational age(P<0.05).Conclusion Gestational age increases the maturation of preterm infants’aEEG images,extrauterine life accelerates the maturation of preterm infants’brain,and aEEG is of great value in evaluating the maturity of preterm infants’brain development.
