TK-1、NSE、CYFRA21-1联合在老年肺癌诊断中的应用价值探究

摘　　要：目的探究胸苷激酶1(TK-1)、神经元特异性烯醇化酶(NSE)、细胞角蛋白19片段(CYFRA21-1)在肺癌诊断中的应用价值。方法选取2020年1月-2024年6月期间清远市人民医院收治的95例经病理确诊为肺癌的患者作为研究对象。所有患者均接受血清TK-1、NSE、CYFRA21-1检测,以临床综合诊断结果作为金标准,分别评估上述指标单独检测和联合检测在肺癌中的诊断效能。采用Kappa值评估各检测方法与金标准之间的一致性。此外,对比肺癌阳性及阴性人群上述血清学指标的差异。结果在95例确诊肺癌患者中,临床综合诊断阳性60例,阴性35例。TK-1检测的Kappa值为0.553,NSE检测的Kappa值为0.687,CYFRA21-1检测的Kappa值为0.539,均表现出一定的一致性。然而,当血清TK-1、NSE、CYFRA21-1联合检测时,Kappa值高达0.978,显示出极高的一致性。联合检测的诊断效能显著优于单独检测,其灵敏度为98.33%,特异度为100.00%,准确度为98.95%,阳性预测值为100.00%,阴性预测值为97.22%,各项指标均高于单独检测,具有统计学意义(P<0.05)。肺癌阳性人群的血清TK-1、NSE、CYFRA21-1水平高于阴性人群(P<0.05)。结论血清TK-1、NSE、CYFRA21-1联合检测在肺癌诊断中具有较高的应用价值,能够显著提高诊断的灵敏度、特异度、准确度和预测值,为肺癌的诊断提供了更为可靠的方法。Objective To investigate the value of thymidine kinase 1(TK-1),neuron-specific enolase(NSE)and cytokeratin 19 fragment(CYFRA21-1)in the diagnosis of lung cancer.Methods Ninety-five patients with pathologically confirmed lung cancer admitted to Qingyuan People’s Hospital from January 2020 to June 2024 were selected as the study objects.All patients received serum TK-1,NSE,and CYFRA21-1 tests.The clinical comprehensive diagnosis results were used as the gold standard to evaluate the diagnostic efficacy of the above indicators in lung cancer respectively.Kappa value was used to evaluate the consistency between the detection methods and the gold standard.In addition,the differences of the above serological indexes between lung cancer positive and negative groups were compared.Results Among 95 patients diagnosed with lung cancer,60 cases were positive and 35 cases were negative.Kappa value detected by TK-1 was 0.553,Kappa value detected by NSE was 0.687,and Kappa value detected by CYFRA21-1 was 0.539,all of which showed certain consistency.However,when serum TK-1,NSE,and CYFRA21-1 were combined,the Kappa value was as high as 0.978,showing high consistency.The diagnostic efficacy of combined examination was significantly better than that of single examination,with sensitivity of 98.33%,specificity of 100.00%,accuracy of 98.95%,positive predictive value of 100.00%and negative predictive value of 97.22%,all of which were higher than that of single examination(P<0.05).The levels of TK-1,NSE and CYFRA21-1 in patients with positive lung cancer were higher than those in patients with negative lung cancer(P<0.05).Conclusion The combined detection of serum TK-1,NSE and CYFRA21-1 has high application value in the diagnosis of lung cancer,which can significantly improve the sensitivity,specificity,accuracy and predictive value of diagnosis,and provide a more reliable method for the diagnosis of lung cancer.
