脑电仿生电刺激联合任务导向性训练治疗痉挛型脑瘫患儿的效果

摘  要:目的:观察脑电仿生电刺激联合任务导向性训练治疗痉挛型脑瘫(SCP)患儿的效果。方法:选取2021年9月至2023年9月该院收治的78例SCP患儿进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各39例。对照组给予任务导向性训练治疗,观察组在对照组基础上联合脑电仿生电刺激治疗。比较两组治疗前后肢体痉挛程度[临床痉挛指数(CSI)]、下肢粗大运动功能[粗大运动功能测量量表(GMFM-88)]、平衡能力[Berg平衡量表(BBS)]和日常生活能力[Barthel指数(BI)]评分。结果:治疗后,两组上、下肢等CSI评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组D区站立、E区走跑跳评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组BBS评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组BI评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:脑电仿生电刺激联合任务导向性训练治疗SCP患儿可提高下肢粗大运动功能、平衡能力和日常生活能力评分,以及降低肢体痉挛程度评分的效果优于单纯脑电仿生电刺激治疗。Objective:To observe effects of electroencephalogram(EEG)bionic electrical stimulation combined with task-oriented training in treatment of children with spastic cerebral palsy(SCP).Methods:A prospective study was conducted on 78 children with SCP admitted to this hospital from September 2021 to September 2023.According to the random number table method,they were divided into control group and observation group,39 cases in each group.The control group was treated with task-oriented training,while the observation group was treated with EEG bionic electrical stimulation on the basis of that of the control group.The scores of degree of limb spasm[clinical spasm index(CSI)],the lower limb gross motor function[gross motor function measurement scale(GMFM-88)],the balance ability[Berg balance scale(BBS)],and the activities of daily living[Barthel index(BI)]were compared between the two groups before and after the treatment.Results:After the treatment,the CSI scores of upper and lower limbs in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The scores of D-zone standing and E-zone walking,running and jumping in the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The BBS scores of the two groups were higher than those before the treatment,that in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).Further,the BI scores of the two groups were higher than those before the treatment,that in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusions:EEG bionic electrical stimulation combined with task-oriented training in the treatment of the SCP children can improve the gross motor function scores of l
