血清IL-33、sFLT1、HE4水平联合检测在子宫内膜癌患者化疗无效中的预测价值

摘　　要：目的:分析血清白细胞介素-33(IL-33)、可溶性血管内皮生长因子受体1(sFLT1)、人附睾蛋白4(HE4)水平联合检测在子宫内膜癌(EC)患者化疗无效中的预测价值。方法:回顾性分析2020年8月至2022年7月该院收治的67例EC患者的临床资料,作为研究组;另回顾性分析该院67名健康体检者的临床资料,作为对照组。比较两组、不同化疗效果患者血清IL-33、sFLT1、HE4水平,绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),分析血清IL-33、sFLT1、HE4水平单项及联合检测在EC患者化疗无效中的预测价值。结果:研究组IL-33、HE4水平高于对照组,sFLT1水平低于对照组,差异均有统计学意义(P<0.05);化疗无效患者血清IL-33、HE4水平高于化疗有效患者,sFLT1水平低于化疗有效患者,差异均有统计学意义(P<0.05);ROC曲线分析结果显示,血清IL-33、sFLT1、HE4水平单项及联合检测预测EC患者化疗无效的AUC分别为0.725、0.738、0.756、0.915,联合检测的预测价值高于三者单项检测。结论:血清IL-33、sFLT1、HE4水平联合检测预测EC患者化疗无效的价值高于三者单项检测。Objective:To analyze predictive value of combined detection of serum interleukin-33(IL-33),soluble vascular endothelial growth factor receptor 1(sFLT1)and human epididymis protein 4(HE4)levels in endometrial cancer(EC)patients with ineffective chemotherapy.Methods:The clinical data of 67 patients with EC admitted to this hospital from August 2020 to July 2022 were retrospectively analyzed and they were set as the study group;the clinical data of 67 healthy subjects in the hospital were retrospectively analyzed and they were used as the control group.The levels of serum IL-33,sFLT1 and HE4 were compared between the two groups and among the patients with different chemotherapy effects.The receiver operating characteristic(ROC)curve was drawn,the area under the curve(AUC)was calculated,and the predictive value of single and combined detection of serum IL-33,sFLT1 and HE4 levels in the EC patients with ineffective chemotherapy was analyzed.Results:The levels of IL-33 and HE4 in the study group were higher than those in the control group,the level of sFLT1 was lower than that in the control group,and the differences were statistically significant(P<0.05).The levels of serum IL-33 and HE4 in the patients with ineffective chemotherapy were higher than those in the patients with effective chemotherapy,the level of sFLT1 was lower than that in the patients with effective chemotherapy,and the differences were statistically significant(P<0.05).The ROC curve showed that the AUC of single and combined detection of serum IL-33,sFLT1 and HE4 levels in the prediction of ineffective chemotherapy in the EC patients were 0.725,0.738,0.756 and 0.915,separately,and the predictive value of combined detection was higher than that of single detection.Conclusions:The value of combined detection of serum IL-33,sFLT1 and HE4 levels in the prediction of ineffective chemotherapy in the EC patients is higher than that of single detection of the three.
