12例胎粪性腹膜炎患儿的产前超声影像、孕期诊治及分娩结局情况

摘　　要：目的探讨胎粪性腹膜炎(MP)患儿的产前超声影像、孕期诊治、分娩结局和新生儿预后情况,以提高对该病的诊疗水平。方法回顾性分析2010年1月至2022年12月首都医科大学附属北京妇产医院诊断的12例MP患儿及母亲的临床资料,分析其孕期影像学特点、孕期诊治、分娩结局及新生儿预后相关情况。结果12例患儿发现影像学异常时的孕龄为21~36周,最常见的超声异常表现胎儿腹腔积液、肠管扩张、肠管强回声;7例染色体及基因拷贝数检查未见异常;6例行宫内治疗,减少胎儿腹腔积液和/或羊水;8例健康存活,智力及生长发育符合年龄。结论产前超声检查对诊断MP有重要意义,加强新生儿围手术期管理,生后积极手术治疗有助于预后。Objective To investigate the prenatal ultrasonography,prenatal diagnosis and treatment,delivery outcomes,and neonatal prognosis of children with meconium peritonitis(MP),in order to improve the level of diagnosis and treatment of MP.Methods Clinical data of 12 children with MP and their mothers diagnosed in Beijing Obstetrics and Gynecology Hospital,Capital medical University from January 2010 to December 2022 were retrospectively analyzed.The imaging features,prenatal diagnosis and treatment,delivery outcome,and neonatal prognosis were analyzed.Results The gestational age of 12 cases was 21-36 weeks,and the most common ultrasonographic abnormalities showed fetal ascites,intestinal dilatation,and strong intestinal echo;seven cases chromosome and gene copy number were no abnormality;six cases intrauterine therapy to reduce fluid accumulation and/or amniotic fluid in the fetal cavity;eight cases survived healthy,and their intelligence and growth were consistent with their age.Conclusion Prenatal ultrasonography is of great significance for the diagnosis of MP.Strengthening perioperative management of neonates and active postoperative surgical treatment are helpful for prognosis.
