腹腔镜子宫内膜癌根治术后患者下尿路症状发生情况及影响因素分析

摘　　要：目的探讨腹腔镜子宫内膜癌根治术后患者下尿路症状发生情况及影响因素。方法将80例行腹腔镜子宫内膜癌根治术的子宫内膜癌患者根据术后是否出现下尿路症状分为下尿路症状组(n=11)和未发生下尿路症状组(n=69),采用Logistic回归模型分析腹腔镜子宫内膜癌根治术后患者发生下尿路症状的影响因素。结果两组患者年龄、尿管留置时间、术中损伤盆腔内脏神经情况、术中损伤膀胱或尿道情况比较,差异均有统计学意义(P﹤0.05)。Logistic回归分析结果显示,尿管留置时间长、术中损伤盆腔内脏神经均为腹腔镜子宫内膜癌根治术后患者发生下尿路症状的独立危险因素(P﹤0.05)。结论腹腔镜子宫内膜癌根治术后患者可能会出现尿路感染、尿失禁、尿潴留等下尿路症状,尿管留置时间长、术中损伤盆腔内脏神经均为腹腔镜子宫内膜癌根治术后患者发生下尿路症状的独立危险因素。Objective To explore the occurrence and influencing factors of lower urinary tract symptom(LUTS)in patients after laparoscopic radical resection of endometrial cancer(EC).Method A total of 80 patients with EC who received laparoscopic radical resection were divided into LUTS group(n=11)and non-LUTS group(n=69)according to whether LUTS occured or not.Logistic regression model was used to analyze the influencing factors of LUTS in patients after laparoscopic radical resection of EC.Result There were statistically significant differences in age,indwelling catheter time,intraoperative injury to pelvic visceral nerves,and intraoperative injury to the bladder or urethra between the two groups(P<0.05).Logistic regression analysis revealed that long catheter indwelling time and intraoperative pelvic visceral nerve injury were independent risk factors for LUTS in patients after laparoscopic radical resection of EC(P<0.05).Conclusion LUTS such as urinary tract infection,urinary incontinence and urinary retentionmay may occur in patients after laparoscopic radical resection of EC.Long catheter indwelling time and intraoperative pelvic visceral nerve injury are independent risk factors for LUTS after laparoscopic radical resection of EC.
