摘　　要：目的探讨多层螺旋CT(MSCT)对卵巢肿瘤良恶性的鉴别诊断价值。方法选取60例卵巢肿瘤患者,以病理检查结果为金标准,评估MSCT对卵巢肿瘤良恶性的鉴别诊断价值,评估MSCT对卵巢肿瘤类型的诊断符合率。比较良性与恶性卵巢肿瘤患者的MSCT征象。结果病理检查结果显示,60例卵巢肿瘤患者中,恶性36例,良性24例。MSCT诊断卵巢恶性肿瘤的灵敏度为91.67%,特异度为87.50%,准确度为90.00%,阳性预测值为91.67%,阴性预测值为87.50%,MSCT诊断卵巢肿瘤良恶性的结果与病理检查结果的一致性较好(Kappa=0.792)。MSCT对卵巢良性肿瘤诊断符合率最高的是卵巢囊性腺瘤,为91.67%,对卵巢恶性肿瘤诊断符合率最高的是卵巢癌,为94.12%。恶性组瘤体形态不规则、增强密度不均匀、有肿瘤分隔、有肿瘤内乳头状凸起、有胸腔积液比例均明显高于良性组,差异均有统计学意义(P﹤0.01)。结论MSCT对卵巢肿瘤良恶性的鉴别诊断价值较高,且对瘤体形态、内部结构、周围组织关系及转移情况显示也非常清晰。Objective To explore the differential diagnostic value of multi-slice spiral CT(MSCT)for benign and malignant ovarian tumors.Method A total of 60 patients with ovarian tumors were selected,using the pathological examination results as the gold standard,to evaluate the differential diagnostic value of MSCT for benign and malignant ovarian tumors.The diagnostic accuracy of MSCT for ovarian tumor types were evaluated.The MSCT features between benign and malignant ovarian patients were compared.Result Pathological examination results showed that among 60 patients with ovarian tumors,36 cases were malignant and 24 cases were benign.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of MSCT in diagnosing ovarian malignant tumors were 91.67%,87.50%,90.00%,91.67%,and 87.50%,respectively.The consistency between MSCT in diagnosis of benign and malignant ovarian tumors and pathological examination results was good(Kappa=0.792).The highest diagnostic accuracy rate of MSCT for benign ovarian tumors was ovarian cystic adenoma,which was 91.67%,while the highest diagnostic accuracy rate for malignant ovarian tumors was ovarian cancer,which was 94.12%.The proportion of irregular tumor morphology,uneven enhancement density,tumor separation,papillary protrusions within the tumor,and pleural effusion in malignant group were significantly higher than those of the benign group,and the differences were statistically significant(P<0.01).Conclusion MSCT has high differential diagnostic value in benign and malignant ovarian tumors,and also provides clear display of tumor morphology,internal structure,surrounding tissue relationships,and metastasis.
