Locator种植覆盖吸附性全口义齿临床效果的回顾性研究

摘　　要：目的:评价Locator种植覆盖吸附性全口义齿的临床效果。方法:选取无牙颌患者共22例。患者单颌植入2~4枚奥齿泰种植体,连接Locator附着体系统进行种植覆盖吸附性全口义齿修复。对患者佩戴种植覆盖义齿后1年、3年、5年的牙槽骨吸收量、BOP、PD、PLI、咀嚼效率及咬合力进行评估。对原义齿、修复后1周、修复1年、修复5年后的患者满意度进行评价。结果:佩戴种植覆盖义齿后的1年、3年、5年MBL存在统计学差异(P<0.05)。无论是修复后即刻、1年,还是5年,咀嚼效率均优于种植修复前的原义齿(P<0.05)。但修复后即刻、修复后1年、修复后5年三者间咀嚼效率无统计学差异(P>0.05)。对患者进行满意度调查,在固位力、咀嚼功能、稳定性方面,各复诊时间点的满意度均优于原义齿(P<0.05),修复后1周的评价与修复后1年无差异(P>0.05),其余均存在统计学差异(P<0.05);在舒适度及美观程度方面,各复诊时间点的满意度均优于原义齿(P<0.05),且各时间点间的满意度无统计学差异(P>0.05)。修复1年与修复3年后的BOP无统计学差异(P>0.05),其余各时间点间的BOP存在统计学差异(P<0.05);各复诊时间点的PD存在统计学差异(P<0.05);各复诊时间点的PLI不存在统计学差异(P>0.05)。各复诊时间点的咬合力均优于原义齿(P<0.05),修复后1年与修复后5年的咬合力无差异(P>0.05),修复后即刻与修复后1年、修复后即刻与修复后5年的咬合力均存在统计学差异(P<0.05)。结论:Locator种植覆盖吸附性全口义齿应用于无牙颌患者,拥有良好的临床修复效果,较传统全口义齿体验感更佳。Objective:To evaluate the clinical efficacy of implant-supported adsorbing overdentures restoration with Locator attachment.Methods:A total of 22 edentulous patients were reviewed.The patients were implanted with 2-4 Ossetian implants and connected to the Locator attachment system for restoration of the overdenture.This research aim to evaluate the MBL,BOP,PD,PLI,masticatory efficiency and occlusal force of patients after wearing implant-supported adsorbing overdentures for 1,3,and 5 years respectively.Satisfaction degree of the retention,masticatory function,stability,comfort level,and aesthetics of the original denture,1 week after restoration,1-year and 3-year repaired were compared.Results:After wearing implant-supported adsorbing overdentures for 1,3,and 5 years,MBL increased year by year,and there was a statistical difference among the three(P<0.05).Whether immediately,1,or 5 years after restoration,masticatory efficiency was better than that of the original complete denture(P<0.05).However,there was no statistically significant difference in masticatory efficiency among the three groups(P>0.05).In terms of retention,masticatory function,and stability,the satisfaction degree at each follow-up time point was better than that of the original denture(P<0.05).There was no significant difference in the satisfaction degree between 1 week after restoration and 1 year after restoration(P>0.05),while there were significant differences in the rest(P<0.05).In terms of comfort level and aesthetics,the satisfaction degree at each follow-up time point were better than that of the original complete denture(P<0.05),and there were no statistically significant difference in each time point(P>0.05).There was no statistically significant difference(P>0.05)in BOP between 1-year and 3-year repaired,while there were statistically significant differences(P<0.05)in BOP in other time points.There were statistical differences in PD at each follow-up time point(P<0.05).There were no statistical differences in PLI at each follow-up tim
