骨性Ⅱ类2分类患者切牙引导角和平面角与颞下颌关节形态相关性的研究

摘  要:目的:探讨骨性Ⅱ类2分类低角患者切牙引导角和平面角与颞下颌关节形态之间的相关性。方法:对52例成人患者的CBCT图像进行分析,其中研究组为25例骨性Ⅱ类2分类低角患者,对照组为27例骨性Ⅰ类均角患者。Invivo Dental 5软件获取切牙引导角(incisal guidance angle,IGA)、平面角(occlusal plane angle,FH-OP)、前部平面角(anterior occlusal plane angle,FH-AOP)及颞下颌关节等测量指标。结果:研究组IGA、FH-OP、FH-AOP均大于对照组(P<0.05)。两组间髁突内外径、髁突头宽度、关节结节高度和倾斜度、关节后间隙差异具有统计学意义,而髁突前后径、髁突倾角、髁突头高度、髁突长度、关节窝深度和宽度差异均无统计学意义(P>0.05)。研究组中IGA与FH-AOP呈中等相关、与FH-OP呈弱相关,与髁突内外径具有弱相关性;而FH-AOP与FH-OP和颞下颌关节指标之间具有相关性。结论:骨性Ⅱ类2分类低角患者切牙引导角不仅与FH-AOP和FH-OP相关,而且与髁突内外径也具有相关性,而平面角与颞下颌关节(temporomandibular joint,TMJ)形态之间存在相关性。Objective:The correlations between the incisal guidance angle,occlusal plane angles and the temporomandibular joint morphology were investigated in adults with low angle skeletal ClassⅡdivisionⅡmalocclusion.Methods:CBCT images of 52 patients were analyzed.It included 25 cases of skeletal Class II divisionⅡmalocclusion with low angle(study group)and 27 cases of skeletal ClassⅠaverage angle(control group).The Invivo Dental 5 software was employed to acquire the data of the incisal guidance angle(IGA),occlusal plane angle(FH-OP),anterior occlusal plane angle(FH-AOP),and the TMJ measurement items.Results:The values of IGA,FH-OP angle,and FH-AOP angle were significantly greater in the study group than those in control group(P<0.05).There were statistical difference in the condylar mediolateral diameters,condylar head width,articular eminence inclination and height and posterior joint spaces between two groups.No differences were revealed in the condylar anteroposterior diameters,the condylar inclination angle,condylar head height,condylar length,glenoid fossa depth and width between two groups(P>0.05).In the study group,IGA showed a moderate correlation with FH-AOP,a weak correlation with FH-OP and condylar mediolateral diameters.While,there was a correlation between FHAOP,FH-OP,and temporomandibular joint indicators.Conclusion:The incisal guidance angle was not only related to FHAOP and FH-OP,but also to the condylar mediolateral diameters in patients with skeletal Class II division II malocclusion.And there was a correlation between the occlusal plane angles and TMJ morphology.
