摘  要:目的:通过锥形束CT(cone-beam CT,CBCT)分析、评价无托槽矫治技术辅助前牙冠延长的效果以及可行性。方法:根据纳入排除标准,从2022年1月至2023年6月于联勤保障部队第988医院口腔科就诊患者中选择10例前牙牙体缺损,需行冠延长术的患者。在完善的术前检查和牙体治疗后,通过无托槽矫治技术对其残根进行正畸牵引,直至达到修复标准,并在牵引完成后进行保持。在正畸牵引完成后,通过对被牵引牙术前术后CBCT数据对比分析,评估冠延长效果。通过对比术前术后被牵引牙的探诊深度、牙龈指数及牙齿松动度评估正畸牵引对牙周健康的影响。结果:经过3个月牵引,牙根平均牵出长度为(2.58±0.51)mm,牵出体积为(25.68±7.50)mm^(3),牵引前后骨内牙体组织的体积、长度差异有统计学意义(P<0.05),患者牵引前后牙周探诊深度减小,减小值为(0.18±0.16)mm(P<0.05),其余测量数据差异无统计学意义(P>0.05)。结论:无托槽矫治技术辅助牵引冠延长的效果能满足修复需要,且对牙周组织健康影响较小,建议临床实践中推广。Objective:To analyze and evaluate the efficacy and feasibility of crown lengthening assisted by bracketless orthodontic technique using cone-beam computed tomography(CBCT).Methods:Based on predefined inclusion and exclusion criteria,10 patients with anterior tooth defects requiring crown lengthening were selected from the Department of Stomatology at the 988th Hospital of the PLA Joint Logistic Support Force between January 2022 and June 2023.After preoperative examination and root canal therapy,orthodontic extrusion was performed on the residual roots using the bracketless orthodontic technique until prosthetic standards were met,followed by retention.The effect of crown lengthening was evaluated by comparing preoperative and postoperative CBCT data,while the impact on periodontal health was assessed through changes in probing depth,gingival index,and tooth mobility.Results:Following a 3-month extrusion period,the root extrusion length was(2.58±0.51)mm,and the extrusion volume was(25.68±7.50)mm.Statistically significant differences were observed in both the volume and length of intra-bone dental tissue before and after extrusion(P<0.05).Probing depth decreased by(0.18±0.16)mm postextrusion(P<0.05),while no significant changes were noted in other measured parameters(P>0.05).Conclusion:Crown lengthening assisted by bracketless orthodontic technique effectively meets prosthetic requirements and has little impact on periodontal tissue health,thus supporting its clinical application.
