摘　　要：目的 :探讨99m锝 甲氧基异丁基异腈 (99m　Tc MIBI)单光子发射型计算机断层显像仪 (SPECT)核素显像对肺癌纵隔淋巴结转移的价值 ,并与CT进行对比。方法 :原发性肺癌 2 2例 ,病人在手术前同时做了CT和99mTc MIBISPECT显像 ,术后进行了病理检查。结果 :99m　Tc MIBISPECT诊断的灵敏性 63 .6% ,特异性 81.8% ,准确性 72 .7%。CT灵敏性 2 7.2 % ,特异性 72 .7% ,准确性 67.7%。结论 :本研究发现99m Tc MIBISPECT核素显像技术在纵隔淋巴结转移检查方面比CT更灵敏、更精确 ,今后应该在应用99m Tc MIBISPECT寻找淋巴结转移方面 ,如乳腺癌、甲状腺癌、头颈部肿瘤等更加广泛的研究 。Objective: Our purpose was to evaluate 99m Tc sestamibi single photo emmision compute tomography(SPECT) in the examination of patients with mediatinal lymphnode metastasis of lung cancer. The results were compared with that of computed tomography(CT). Methods: Twenty two patients with proved lung cancer were included in this study. All patients received examinations with CT and 99m Tc MIBI SPECT for thorax for presurgical staging . Postsurgical pathologic diagnosis was performed, and these patients were evaluated for mediastinal lymphnode metastasis. Results: Examination with 99m Tc MIBI SPECT showed a diagnostic sensitivity of 63.6%, specificity of 81.8%, and accuracy of 72.7 %. Results of CT gave a sensitivity of 27.2%, specificity of 72.7%, and accuracy of 67.7%. Conclusion: We found that 99m Tc MIBI SPECT was more sensitive and accurate than CT in the assessment of mediastinal lymphnode metastasis for patients with lung cancer patients; however, more extensive studies should be performed to confirm the clinical role of 99m Tc MIBI SPECT.
