针吸细胞学检查在乳腺癌诊断中的意义

摘　　要：目的 :观察乳腺肿物针吸细胞学诊断与组织病理学检查的符合率。方法 :对 1 66例乳腺肿物吸出组织 ,经巴氏 ( Papanicolaou)染色 ,参照分级标准及良、恶性肿瘤组织细胞的形态进行显微镜下早期诊断。结果 :1 66例针吸细胞学诊断与组织病理检查诊断符合率为 89.76%、灵敏度 84.81 %、特异度 94.2 5 %。其中良性肿瘤 69例 ,假阴性 1 2例 ( 1 2 .77% ) ;乳腺癌和高度怀疑有癌特征的 97例 ,假阳性 5例 ( 6.94% )。结论 :乳腺针吸细胞学检查在乳腺肿物的临床诊断中具有准确、可靠、安全、经济等重要意义 ,可以作为乳腺癌诊断的重要手段。Objective:To study the coincidence rate of the fine-needle aspiration cytohistology and biopsy histopathology in the diagnosis of breast tumor.Methods:Fine-needle aspiration cytohistology in 166 cases with breast tumor,which stained by the Papanicolaou staining,classed and diagnosed under microscope by benign and malignant tumor standar in the earlier period.Results:The coincidence rate was 89.76%,sensitivity was 84.81%,specificity was 94.25%,in the diagnosis of breast tumor with the fine-needle aspiration cytohistology and histopathology.The false-negative was 12 cases among 69 cases of benign tumor,and the false-positive was 5 cases among 97 cases of malignant tumor and with cancer characteristic tumor.Conclusion:The fine-needle aspiration cytohistology is a exact and safe and effective technique for the clinical diagnosis of breast tumor in the earlier period.
