老年食管癌合并糖尿病的手术危险因素

摘　　要：目的　分析老年食管癌合并糖尿病的手术危险因素。方法　同期老年食管癌病人分为实验组(合并糖尿病 )及对照组 (无糖尿病 ) ,均行手术治疗。观察两组术前、术后心血管疾病发生率及肺部感染、吻合口瘘等并发症发生率。结果　实验组术前、术后心血管疾病发生率分别为 33 33% (8/ 2 4 )、4 5 83% (11/ 2 4 ) ,术后肺部感染发生率 2 5 % (6 / 2 4 ) ,吻合口瘘发生率 12 5 % (3/ 2 4 ) ,与对照组比较明显升高 (P <0 0 5 )。结论　老年食管癌合并糖尿病病人手术危险明显增加 ;老年食管癌合并糖尿病病人心血管疾病发生率明显升高 ,是增加手术危险的重要因素之一。Objective To study the operative risk factors in elderly patients with carcinoma of esophagus complicated DM.Methods One hundred and seventy-five elderly patients with carcinoma of esophagus were classified as the experimental group(24 cases complicated DM) and the control group(151 cases without DM).Occurrence of complications and cardiovascular diseases were observed and compared.Results Significant distinctions were found in incidence of preoperative cardiovascular diseases,postoperative cardiovascular diseases,pulmonary infections,anastomosic leakage between both groups.The P values was <0.05,<0.01,<0.05,<0.05 respectively.Conclusion Operative risk factors in elderly patients with carcinoma of esophagus complicated DM were more than those cases without DM.Incidence of cardiovascular diseases were high in experimental group.The higher incidence of cardiovascular diseases is one of the important risk factors.
