摘　　要：目的　评价自体外周血干细胞移植联合大剂量化疗治疗恶性血液病的近期疗效 .方法　 8例恶性血液病 (非霍奇金淋巴瘤 7例 ,多发性骨髓瘤 1例 )采用化疗联合G CSF动员外周血干细胞 ,CobeSpectra血细胞分离机采集循环血中的单个核细胞 ,8例平均单个核细胞数 3.0× 10 8·kg-1,平均CD34+ 细胞为 3.5× 10 6·kg-1,- 80℃冰箱冻存 .8例均在大剂量化疗后回输细胞 .结果　外周血干细胞回输后WBC恢复至 2× 10 9·L-1平均为 11d ,血小板恢复至 5 0× 10 10 ·L-1平均为 13d .8例移植后短期内均达缓解 .结论　化疗联合G CSF是一高效、低毒的动员方案 ;自体外周血干细胞移植联合大剂量化疗是治疗难治性恶性血液病的一种安全、近期疗效肯定的治疗方法 .AIM To evaluate the protocol for the mobilization and collection of peripheral blood stem cell (PBSC) and to observe the efficiency of autologous peripheral blood stem cell transplantation with high dose chemotherapy for patients with hematologic malignancies. METHODS The peripheral blood stem cells of 8 patients (7 cases of non Hodgkin's lymphoma and 1 case of multiple myeloma) were mobilized with a combination of chemotherapy and G CSF. Mononuclear cell (MNC) apheresis was performed by cell separator Cobe Spectra to collect circulating PBSC and NMCs were cryopreserved at -80℃. The collecting mean number of MNCs and CD34 + cells was 3.0×10 8·kg -1 , and 3.5 ×10 6·kg -1 . Mononuclear cells were reinfused after high dose chemotherapy to reconstitute haemopoiesis. RESULTS After PBSC infusing, the mean time of white blood cell count reaching more than 2×10 9·L -1 and platelet count more than 50×10 9·L -1 was 11 and 13 days, respectively. Eight patients achieved remission after PBSCT. CONCLUSION The combination of chemotherapy and G CSF is an effective and low toxicity protocol for the mobilization of PBSC. The high dose chemotherapy with autologous peripheral blood stem cell transplantation can effectively treat patients with refractory hematologic malignancy.
