摘　　要：目的 :探讨肾囊内甲基强的松龙注射治疗狼疮性肾炎 (LN)的临床效果。方法 :2 0例经肾活检证实为狼疮性肾炎的病人 ,B超引导下向每侧的肾脂肪囊内注射甲基强的松龙 4 0mg ,每周 2次 ,共 10次 ,同时配合环磷酰胺 (CTX) 1g加液体静滴 ,每月 1次 ,治疗前后观察病人的 2 4h尿蛋白定量 ,血压 ,血肌酐 ,血糖。结果 :2 0例病人中 17例病人尿蛋白明显减少或转阴性 ,未发现明显糖皮质激素的副作用。结论 :肾囊内注射甲基强的松龙可有效治疗狼疮性肾炎 ,同时可以避免或减少全身大量应用激素带来的副作用。配合应用环磷酰胺减少了狼疮活动和复发。Objective: To study the treatment that methyprednisolone was injected into renal adipose capsuies of patients to treat lupus nephritis. Methods: 20 patients were dignosed as lupus nephritis by renal biopsies, under the guide of sonograph ,40mg methylprednisolone was injected into each renal adipose capsule of the patients twice a week ,and everyone was totally given ten times of the treatment .in this time the patients was given CTX 1g iv drip once a month. the urinary protein ,plasma albumin, serum creatinine blood pressure and blood glucose,before and after the treatment were measured. Results: The excertion of urinary protein in 17 of 20 patients had significantly been decreased , and even urinary protein of some patients were negative. we did not find any obvious side effects of glucocorticoid. Conclusions: The intra-renal adipose capsule injection methyprednisolone is an effective procedure to treat lupus nephritis with much fewer side effects. lupus activets and repeat ruduced after useing CTX.
