摘  要:目的 分析 A、B超联合法测眼轴计算高度近视眼并发白内障人工晶状体度数的准确性。方法 根据测得的眼轴长度把所有病例分为三组 :(1)眼轴长 2 6~ 2 8m m组 :11只眼 ;(2 )眼轴长 >2 8~ 30 mm组 :15只眼 ;(3)眼轴长 >30 mm组 :17只眼。另选择眼轴为 2 3~ 2 4 m m的 15只眼作为对照 ,用两种方法测量眼轴计算人工晶状体度数。方法一 :A、B超联合测法。首先用 A超测量眼轴 ,探头轻轻置于瞳孔中央与角膜顶点的切线垂直。系统自动重复测量十次。取前房深度及晶状体厚度的平均值之和定为 DA。再行 B超检查 ,观察轴位图。用 B超上的 A超测量尺测量后囊中心到视乳头颞侧 3m m中心凹处的距离 ,值定为 DB。DA+DB即为 A、B超联合测量方法测得的眼轴长度。方法二 :单纯 A超测量法 ,即直接使用 A超测量眼轴长度。采用 SRK 公式计算出所需的人工晶状体度数。结果  A、B超联合测量法为 :术后 3个月实际屈光度与预计屈光度的差值在眼轴长 2 6~ 2 8mm组为 0 .2 3±0 .4 5 D;眼轴长 >2 8~ 30 mm组为 - 0 .35± 0 .5 9D;眼轴长 >30 mm组为 - 0 .5 9± 0 .79D。单纯用 A超测量法结果为 :眼轴长 2 6~ 2 8mm组为 0 .4 5± 0 .90 D;眼轴长 >2 8~ 30 mm组为 0 .97± 1.2 7D;眼轴长 >30 mm组为 1.4 5± 2 .0 4 D。经 t检?Objective To analyze the accuracy of measurement for the diopter of artificial lens in high myopia with cataract by A scan combined with B scan.Methods According to axial length,43 high myopia with cataract cases were divided into three groups.11 eyes were in 26~28mm axial length group,15 eyes in 28~30mm axial length group,and 17 eyes in >30mm axial length group.Two different methods,A Scan combined with B Scan and only A scan,were used to measuring the axial length,and the axial length and the postoperative refractive diopter were compared.Results The difference between acutal and anticipated refractive dioper in three groups were 0.23±0.45D,-0.35±0.59D and -0.59±0.75D by A scan combined with B scan,and 0.45±0.90D,0.97±1.27D,1.45±2.04D only by A scan respectively.There were statistically significant difference betwee two methods.Conclusion To measure the diopter of artificial lens in high myopia with cataract,the method of A scan combined with B scan is more precise than that of only by A scan.
