多层螺旋CT胰周血管成像对胰腺癌可切除性评估的应用

摘　　要：目的研究胰腺癌患者使用多层螺旋CT胰周血管成像对可切除性评估的临床价值。方法整群选取2012年8月—2014年8月期间收治的68例胰腺癌患者,所有患者均采用手术病理与CT扫描检查胰腺癌对胰周血管的侵犯情况。结果对胰腺血管进行CT探查Loyer分型,灵敏度为84.79%,特异度为72.46%;对胰腺周围主要动静脉进行CT探查血管分级,灵敏度为90.78%,特异度为73.91%。结论多层螺旋CT胰周血管成像对胰腺癌患者可切除性评估灵敏度及特异度均较高,有着重要临床价值,值得临床应用及推广。Objective To study the clinical application value of multi-slice spiral CT angiography of peripancreatic vessel in the resectable assessment of pancreatic cancer. Methods 68 patients with pancreatic cancer treated from August 2012 to August 2014 were selected. The pathological examination and CT scan was applied to understand the invasion of pancreatic cancer in the peripancreatic vessel. Results Based on CT scan on Loyer type of pancreatic vessel, the sensitivity was 84.79% and the specificity was72.46%; based on CT scan on the classification of blood vessel for the major arteries and veins of peripancreatic vessel, the sensitivity was 90.78% and the specificity was 73.91%. Conclusion The multi-slice spiral CT angiography of peripancreatic vessel has a higher sensitivity and specificity in the resectable assessment of pancreatic cancer. It is of great significance and worthy of clini-cal application and promotion.
