摘 要:目的 :观察对比气药灌肠和常规灌肠治疗溃疡性结肠炎 (UC)的疗效 ,并探讨一氧化氮 (NO)与UC的发病及治疗转归的关系。方法 :60例UC患者分为常规灌肠组 ( 32例 )和气药灌肠组 ( 2 8例 ) ,气药灌肠组采用电脑灌肠仪 ,利用气压将药液注入结肠。采用RT PCR技术检测气药灌肠组患者治疗前后肠黏膜组织的诱导型一氧化氮合酶 (iNOS)mRNA表达情况 ,用硝酸还原酶法测定两组患者治疗前后血清NO水平。结果 :气药灌肠疗法对UC的脓血便、黏液便和溃疡病变的治疗效果优于常规灌肠方法。随着病情好转 ,溃疡处肠黏膜iN OSmRNA表达量和血清NO水平下降。结论 :气药灌肠方法能将药液均匀分布于整个结肠 ,提高对UC的治疗效果 ;NO可能参与了UC的病变过程 。Objective To observe the effect of air-pressured herb enema versus routine way and to discuss the relationship between nitrous oxide(NO) and ulcerative colitis(UC).Methods Enema was performed in 60 patients with UC either regularly(n=32) as in control group or with air-pressured herb(n=28) as in test group.In the latter,herb enema was perfused under pressure into colon using computerized enemator.iNOS mRNA of ulcerative mucosa was detected by RTPCR technique for Air-pressured enema group.Serum NO level was detected by nitrate reductase method.Compared the differences of serum NO level of two groups.Results Air-pressured enema was better than the routine way.With the recovering of the illness,iNOS mRNA decreased.Conclusion Air-pressured enema increases the concentration of herbs in colitis and improves the treatment effect of UC.NO,involved in the development of UC,is related to the recovery of UC.
