FOLFOX方案治疗30例晚期胃癌疗效观察

摘　　要：目的　观察每二周高剂量亚叶酸钙 (CF) /氟脲嘧啶 (5 FU )与草酸铂 (L OHP)方案(FOLFOX方案 )治疗晚期胃癌的临床疗效和毒副反应。方法　采用高剂量CF/ 5 FU/L OHP深静脉输注方案 (CF2 0 0mg·m2 -1·d-1,静滴 2小时 ,第 1、2天 ;5 FU40 0mg·m2 -1·d-1,静推 ,第 1天 ;5 FU160 0mg·m2 -1·d-1,静滴 2 2小时 ,第 1、2天 ;L OHP13 0mg·m2 -1·d-1,静脉输注 4小时 ,第 1天 ) ,化疗方案以 14天为 1周期 ,重复 4周期后间隔 1个月评定疗效。结果　全组 3 0例 ,总有效率为5 3 3 %。 18例初治组的有效率为 61 11% ,其中CR1例。 12例复治组的有效率为 41 7%。初治组中位缓解期为 5个月。复治组的中位缓解期为 3个月。Ⅱ、Ⅲ度口腔炎发生率为 2 6 7% ,7例出现手足综合征 ,血液学毒性轻微。结论　每二周高剂量CF/ 5 FU/L OHP方案是治疗晚期胃癌有效安全的化疗方案。Objective To evaluate the efficacy and toxicity of the biweekly regimen of high-dose leucovorin (CF),continuous central venous infusion (c.c.v.i)of fluorouracil (5-FU) and oxaliplatin(L—OHP)(FOLFOX)for advanced gastric cancer.Methods 30 patients received biweekly CF/5-FU/L—OHP(CF 200mg·m 2 -1 ·d -1 iv.gtt.2 hour,day1,2;5-FU400mg·m 2 -1 ·d -1 ,i.v.,day1;5-FU 1*!600mg ·m 2 -1 ·d -1 ,i.v.22 hours,day1,2;L—OHP 135mg·m 2 -1 ·d -1 ,iv.gtt.4 hour,day1,every two weeks).Four treating courses were carried out with a one month interval in between.Results The overall response rate was 53 3%.The response rates of patients in primary treatment was 61 1%(1 CR) and that return cases was 41 7%( P> 0 05)。Median duration of partial responses in primary treatmen was 5 0 months and that return cases was 3 0 months.Grade 2 or 3 stomatitis occurred in 26 7% of patients.7patients developed hand-foot syndrome.Hematologic toxicity were minimal.Conclusion The biweekly regimen of high-dose leucovorin /5-fluorouracil and oxaliplatin has a high response rate with tolerable toxicity in patient with advanced gastric cancer
