摘　　要：目的　探讨Cytokeratin18(CK18)蛋白在大肠癌淋巴结微转移 (micrometastases)癌中的表达及其意义。方法　应用免疫组织化学法 (S P法 )检测CK18在 6 0例大肠癌患者淋巴结微转移灶中的表达。分析CK18的表达与HE染色的关系和检出的微转移灶与年龄、性别、病理类型、分化程度、临床分期之间的关系。结果　① 2 2例患者 (36 .6 7% ) 39枚 (16 .70 % )淋巴结存在微转移 ,主要以单个散在和小团状分布于边缘窦内。②CK18蛋白表达阳性与HE染色阳性比较 ,差异有极显著性 (P <0 .0 1)。③淋巴结微转移灶的检出与患者年龄、性别、病理类型、分化程度无相关性 (P >0 .0 5 ) ,与临床Dukes’分期明显相关 (P <0 .0 1)。结论　对大肠癌患者淋巴结微转移进行检测 ,能够更准确的确定临床分期 ,判断预后。在检测淋巴结微转移方面 ,CK18具有较高的敏感性 ,可提高检出率 。Objective To explore the expression and clinical significance of CK18 in lymph node micrometastasis with colorectal carcinoma.Methods Immunohistochemical SP method was used to detect CK18 in lymph node micrometastasis among 60 cases of colorectal carcinoma. The relationship between detected micrometastasis and age, gender, pathological types, differentiation degree, clinical stages, the relationship between CK18 expression and H&E staining were analyzed. Results ① The micrometastasis was typically visible from 39 lymph nodes (16.70%) in 22 cases (36.67%) in single cells or in small clusters of carcinoma cells present in nodal sinuses, mainly in the marginal sinus. ② The expression rate of CK18 was significantly higher than that of H&E staining (P<0.01). ③ The presence of micrometastasis was significantly correlated with clinical stages (P<0.01), but was not correlated with age, gender, pathological types, differentiation degree (P>0.05).Conclusions The detection of lymph node micrometastasis with colorectal carcinoma patients can determine the climical stages and prognosis more accurately. CK18 has a higher sensitivity in the detection of lymph node micrometastasis. The application of it, which increases the detection of carcinoma cells in lymph nodes , can be recommended.[
